About this file

This is a Form 1295 Acknowledgment of Exemptions Form issued by Fort Bend Independent School District (FBISD) in Texas, required under House Bill 1295 to comply with Texas Government Code Section 2252.908 regarding the disclosure of interested parties. The form applies to a solicitation for cheerleader and dance team uniforms, supplies, and related items for students in grades K-12. Any business entity seeking to execute a Board- or Superintendent-approved contract with FBISD must submit a completed Disclosure of Interested Parties (Form 1295) unless the contract qualifies for an exemption.

The form provides six exemption categories that vendors may claim if applicable: contracts with institutions of higher education for sponsored research; interagency contracts of state agencies or higher education institutions; health and human services contracts where value cannot be determined at execution and any qualified vendor is eligible; contracts with publicly traded business entities or their wholly owned subsidiaries; contracts with electric utilities as defined by Section 31.002 of the Texas Utilities Code; and contracts with gas utilities as defined by Section 121.001 of the Texas Utilities Code. Vendors must certify the accuracy of information provided, acknowledge that failure to comply with Texas Government Code 2252.908 may result in contract delays or termination, and submit the completed form through the Bonfire procurement platform in the Required Forms section.
