About this file
This is a Change Order Notice issued by the Chief Procurement Office for General Services on behalf of the Illinois Department of Public Health (IDPH) for Hologic NG/NT Testing and Kits under BidBuy Reference Number 27-482DPH-OFCDI-B-53956. The change order addresses a publication error in the original Invitation for Bid (IFB) process for Contract 68500013N and realigns contract quantities to reflect actual programmatic needs for statewide sexually transmitted infection (STI) testing. The Carbondale Laboratory, which conducts Neisseria gonorrhoeae (NG) and Chlamydia trachomatis (CT) screening using the Hologic Panther NAAT platform under a reagent rental agreement, requires increased annual testing volumes. The original IFB incorrectly listed annual quantities at 15,000 NG/CT tests and 15,000 collection kits per year. The amended contract increases the annual testing volume to 25,000 tests (an additional 10,000 tests per year) and the annual kit volume to 30,000 collection kits (an additional 15,000 kits per year) to meet actual laboratory demand.
The original contract maximum was $525,000, with the change order increasing the cost by $375,000 (an additional $75,000 per year), resulting in a revised total contract cost of $900,000 over the remaining five-year initial term with potential for a ten-year contract duration. All other contract terms, conditions, unit pricing, and vendor obligations remain unchanged, including the reagent rental structure that provides free instrumentation, preventive maintenance, repairs, replacement parts, installation, training, and technical support. The change order was required to ensure sufficient supplies and uninterrupted statewide STI testing operations and meets the threshold for posting on BidBuy for 14 days as it increases contract cost by more than $25,000.

ProcurementTitle: CHANGE ORDER NOTICE Hologic NG/NT Testing and Kits
BBREFNO.: 27-482DPH-OFCDI-B-53956
thedetails: This change order is required to correct a publication error made during the original Invitation for Bid (IFB) process and to align the contract quantities with actual programmatic needs for statewide sexually transmitted infection (STI) testing. When Contract 68500013N was bid and awarded, the annual quantities published in BidBuy for NG/CT test kits and collection devices were inadvertently listed incorrectly. Although the IFB stated that all quantities were estimates for bidding purposes only, the published numbers did not reflect the actual laboratory testing volumes required by the Illinois Department of Public Health (IDPH) Carbondale Laboratory.
The Carbondale Laboratory conducts statewide Neisseria gonorrhoeae (NG) and Chlamydia trachomatis (CT) screening using the Hologic Panther NAAT platform provided under a reagent rental agreement. This amendment updates the contract quantities to reflect actual testing demand, which is significantly higher than the 15,000 NG/CT tests and 15,000 collection kits per year originally listed in the IFB publication. The laboratory requires an additional 10,000 NG/CT tests per year, thereby increasing the annual testing volume from 15,000 to 25,000. It also requires an additional 15,000 collection kits per year, thereby increasing the annual kit volume from 15,000 to 30,000. The increased annual quantities result in an additional $75,000 per year, totaling $375,000 over the remaining five year initial term.
This change order updates the contract maximum, corrects the annual quantities of reagents and collection devices, and revises the scope to reflect the required volumes. These corrected quantities ensure sufficient supplies and uninterrupted statewide STI testing operations for the full potential ten year contract duration.
Original Cost: $525,000.00 Amount of Change: $375,000.00 Revised Cost: $900,000.00
