About this file

This document is a Contract Data Requirements List (DD Form 1423) that specifies data deliverables required under a Department of Defense contract for system 4T-1356-01-106-5985 WW98. The form identifies five distinct data items with specific submission requirements, approval authorities, and distribution parameters.

The five contract line items are: (1) Test Procedure (A001) requiring one-time submission within 60 days of award with approval required before final submission; (2) Test/Inspection Report (A002) for pre-production and production testing with submission 15 days after first article test completion and subsequently 15 days after each lot acceptance; (3) Failure Analysis/Corrective Action Report (A003) requiring submission as interim reports within 10 days of lot failure notification with final reports due 30 days after failure notification; (4) Ammunition Data Card (A004) prepared per MIL-STD-1168 with monthly submissions to NAVSEA PMS-495 and government inspector signature requirements; and (5) Contractor's Progress, Status and Management Report (A005) requiring monthly submissions via email or mail by the fifth day of each month. All data items specify distribution to identified points of contact, with copies required in both draft and final formats except where noted. Instructions for completing the form are provided for government personnel and contractors, detailing pricing group classifications (Groups I-IV) and guidance on estimating data development costs.
