About this file
This is a Debarment and Suspension Certification form required by Grand Rapids Public Schools for bidders responding to the eRate 2026 Network Switching Electronics RFQ in Michigan. The certification serves as a mandatory compliance document that bidders must complete and submit as part of their bid submission. Under penalty of perjury, the bidder and any associated persons in ownership, partnership, directorship, officer, or management capacities must certify their compliance with federal debarment and suspension requirements. The bidder must confirm they are not currently under suspension, debarment, voluntary exclusion, or determination of ineligibility by any Federal agency; have not been suspended, debarred, voluntarily excluded, or determined ineligible by any Federal agency within the past three years; do not have a proposed debarment pending; and are not presently indicted for or otherwise criminally or civilly charged by any governmental entity at the Federal, State, or Local level in any matter involving fraud or official misconduct within the past three years.
Bidders are permitted to note any exceptions to the certification in a designated space on the form, though exceptions will not necessarily result in bid denial or award denial but will be considered in determining bidder responsibility. For any exceptions noted, bidders must identify the party to whom it applies, the initiating agency, and the dates of action. The form requires signature and title from the authorized representative, as well as the company name and date. False information provided on this certification may result in criminal prosecution or administrative sanctions. The signature on this certification document constitutes the bidder's signature on the bid itself.
