Attachment C – Verification Form Summary

This is a verification form attachment associated with the City of Liberty Hill's on-demand public transportation rideshare program solicitation in Texas. The form serves as a compliance and attestation document for respondents participating in this contract opportunity. The City seeks a qualified individual, firm, or corporation to develop, implement, administer, and operate a comprehensive on-demand public transportation rideshare program serving residents and visitors within a defined service area. The program requires a turnkey solution integrating standard on-demand rideshare service with complementary ADA-compliant wheelchair accessible vehicle (WAV) service, multiple rider access methods, data reporting capabilities, community outreach, and ongoing program administration under a single prime contract. The City intends to award one contract to the respondent whose proposal demonstrates the most advantageous combination of experience, service model, accessibility features, implementation capability, reporting infrastructure, and pricing. The City explicitly excludes proposals that include a new City-managed fleet as part of the service package.

Respondents may utilize established third-party rideshare platforms, rider access tools, dispatch systems, and subcontractors to fulfill contract requirements; however, the prime contractor remains solely responsible to the City for implementation, service delivery, subcontractor oversight, compliance, performance reporting, and overall contract administration. The verification form functions as a certification mechanism to ensure respondents meet specified qualifications and agree to contractual obligations before proposal submission.
