About this file

This is a Quality Assurance Surveillance Plan (QASP) for the contract titled "The Literate Brain: Developing Pedagogical Content Knowledge in the Science of Reading & Writing (Grades PK-5)." The QASP establishes procedures for Government oversight of Contractor performance throughout contract execution and connects directly to the Performance Work Statement (PWS). The document designates the Contracting Officer (KO) as the sole individual authorized to make contractual commitments and authorize changes, while the Contracting Officer's Representative (COR) provides continuous technical oversight, maintains quality assurance documentation, and reports performance issues to the KO.

The Performance Requirements Summary Matrix identifies thirteen specific performance objectives with associated standards, Acceptable Quality Levels (AQLs), surveillance methods, and designated inspectors. Key deliverables include: (1) a project implementation plan requiring 100% accuracy in final form; (2) role-specific professional learning content meeting DoWEA-approved methodologies with 95% on-time delivery; (3) facilitation preparation sessions and related support with at least 75% participant satisfaction ratings; (4) evaluation instruments and reporting with quantitative growth patterns and actionable recommendations; (5) comprehensive asynchronous modules maintaining 100% parity with live training; (6) technical support resolving 90% of issues within one business day; (7) digital and print resource delivery with seamless LMS integration and 95% on-time physical shipments; (8) meeting coordination and minutes; and (9) privacy compliance including user accounts management, privacy training, and annual written attestation. The document specifies nonconformance procedures requiring re-performance at no additional cost or potential contract termination for cause if deficiencies persist, and mandates Combatting Trafficking in Persons (CTIP) annual checklist submission.

1) Title/Contract #: THE LITERATE BRAIN: Developing Pedagogical Content Knowledge in the Science of

Reading & Writing (Grades PK-5) (Pending award)

2) Purpose: This QASP is a Government-developed document used to determine if the Contractor’s performance meets the performance standards contained in the contract. The QASP establishes procedures on how this surveillance/ inspection process will be conducted and connects directly with the contract’s Performance Work Statement (PWS), which identifies specific task requirements the Government will focus on during its inspection of services. It provides the process for continuous oversight detailing: 1) what is monitored, 2) how monitoring takes place, 3) who conducts monitoring, and 4) how monitoring efforts and results are documented. The Contractor is responsible for implementing and delivering performance that meets contract standards using its quality control plan. The Government is responsible to be objective, fair and consistent in evaluating Contractor performance. The QASP is not part of the contract nor is it intended to duplicate the Contractor’s quality control plan. This QASP is a living document.

3) Roles and Responsibilities: The following personnel shall oversee and coordinate surveillance activities:

Table 1, Key Personnel

Role Duties Name Phone # Email

Contracting

Officer (KO)

The KO ensures performance of all necessary actions for effective contracting, ensures compliance with the contract terms, safeguards the interests of the

United States in the contractual relationship, and determines the final assessment of Contractor performance.

The KO is the only individual auth-orized to make contractual commit-ments or authorize contractual changes on the Government’s behalf.

Contracting

Officer’s

Representative

(COR)

The COR is responsible for providing continuous technical oversight of the

Contractor’s performance and uses the

QASP to conduct the oversight/ surveillance process. The COR must keep a Quality Assurance file that accurately documents the Contractor’s actual performance. The COR is responsible to report early identification of performance issues to KO. The COR is not empowered to make contractual commitments or authorize contractual changes on behalf of the Government.

4) Performance Requirements Summary Matrix (PRSM): Table 2 below lists the contract’s requirements:

▪ Performance Standards: Defines desired service requirements/outcomes and used to measure the Contractor’s success in achieving them.

▪ Acceptable Quality Level (AQL): Defines the maximum allowable variance from a standard before the Government rejects a service and can be expressed as a number, percentage, or a quantity per number of units inspected.

▪ Method of Surveillance: Details the inspection processes the Government utilizes to ensure quality services are received under the contract.

QUALITY ASSURANCE SURVEILLANCE PLAN

▪ Inspector: The individual responsible for conducting the surveillance.

5) Nonconformance

▪ Nonconformance with contract requirements is termed a “Deficiency”. “Deficiency” refers to performance of a service requirement that does not meet the associated contractual requirement defined in the contract’s PWS.

▪ For services that do not conform to contract requirements, the Government may require the Contractor to re-perform to ensure conformity and acceptance at no additional cost to the Government. In the event substandard services cannot be corrected through re-performance the Government will require the Contractor to either (1) take necessary action to ensure that future performance conforms to contract requirements or (2) reduce the contract price in accordance with KO guidance.

▪ In the event substandard performance occurs, should the Contractor fail to take the necessary action to ensure future conformance with contract requirements, or promptly re-perform services to an acceptable level, the

Government may either (1) have the services performed by other means and charge to the Contractor any cost incurred or (2) terminate the contract for cause.

b. Customer Complaint Form

c. Quality Assurance Monitoring Form

7) Combatting Trafficking in Persons (CTIP) COR Oversight: The Government prohibits trafficking in persons.

CORs are the first line of defense against human trafficking. As such, CORs must be aware of how Contractors treat employees and understand the prohibited activities outlined in the associated contract.

COR Oversight Requirements: The COR must monitor Contractor performance and maintain surveillance over

Contractor compliance with CTIP requirements and immediately report any suspected violation of CTIP policy to the

KO. The CTIP Checklist may be used to report violations.

COR Reporting Requirements: Contractors shall complete the CTIP Checklist annually and submit it to the COR to verify the Contractor is not engaging in CTIP prohibited activities.
