About this file

This is a Bidder's Checklist for solicitation 26HS0001 issued by the California Department of Veterans Affairs for Operating Equipment Maintenance/Repair Services at the Northern California Veterans Cemetery. The checklist outlines all required documents and certifications that bidders must submit with their bids, including a Bid Rate Sheet, signed Payee Data Record (STD 204), Bidder Declaration, and Contractor Certification Clauses. Bidders must also provide proof of business licensure or good standing, such as a valid California city or county business license, incorporation documents for California corporations, or an affidavit of good standing for out-of-state businesses. Additional documentation requirements include Commercially Useful Function (CUF) Certification if applicable, Disabled Veteran Business Enterprise (DVBE) Declarations if applicable, and Darfur Contracting Act compliance documentation.

The checklist emphasizes that all company names must be identical across all submitted documents, including licenses, permits, certifications, and insurance, and must match the company name on the Payee Data Record to avoid delays in contract award. Bidders are required to submit a sample invoice and certify compliance with all applicable State of California standard contract language. The complete scope of work and additional requirements can be found in Exhibit A and the Notice to Prospective Bidders (IFB Letter), and prospective bidders must direct all questions via email according to the specified timeline.
