About this file
This is a Bidder's Checklist document issued by the State of California Department of Veterans Affairs for solicitation number 26FS0005, which concerns Grease Trap Cleaning and Disposal Services at the Veterans Home of California at Fresno. The solicitation requires bidders to submit comprehensive documentation including a Bidder Certification, Bid Rate Sheet, signed Payee Data Record (STD 204), Bidder Declaration, and applicable certifications for Commercially Useful Function and Disabled Veteran Business Enterprise status. Bidders must also provide proof of valid California business licensing, incorporation documents for California corporations, or an affidavit of good standing for out-of-state businesses. Additional required submissions include the Darfur Contracting Act compliance documentation and Contractor Certification Clauses (CCC-4/2017), with a sample invoice template available through the California Department of General Services Office of Legal Services.
The checklist emphasizes that all company names appearing on licenses, permits, certifications, and insurance documentation must match exactly with the company name listed on the Payee Data Record to avoid delays in contract award. Bidders should submit all required documents in the manner prescribed and include a completed checklist with their bid package. Additional details regarding the scope of work, timeline, and submission procedures are contained in the associated Notice to Prospective Bidders and Invitation for Bid documents, with all inquiries to be directed via email as specified in the solicitation materials.
