Required submissions include a quote rate sheet, standard payee data records (STD 204 and STD 205), contractor certification clauses, bidder declarations, DVBE certification forms, proof of required licenses or certifications, and sample invoices. Contractors must provide comprehensive general liability, commercial general liability, automobile liability, excess/umbrella liability, and workers' compensation insurance with the State named as an additional insured. The specific budget amount is not disclosed in the solicitation materials; however, a mandatory deduction of $10,000 (or the full final payment if less) will be imposed for failure to submit the required Prime Contractor DVBE Subcontracting Report upon contract completion. No protest rights are available under this solicitation method.
