PROJECT DETAILS
State the purpose of the project and provide project details for the length of the budget period (11 months).
For the selected program strategies:
1. Describe proposed activities and provide sample process measures to track progress implementing the activities
2. Describe the expected outputs (resources, tools, products, etc.) and outcomes (change to occur), and provide sample outcome measures to track progress achieving the outcomes.
11 Project Purpose : * 134/150 words maximum
12 12.1. Public Health Systems Infrastructure Activities*
The goal of this project is to provide customized systems and performance improvement technical assistance (TA) to small local health departments across the United States that are oftem limited in resources and capacity. Eligible local health departments must be small (defined by serving <49,000 population, and/or having <5 full time health department staff), Health department pursuit of accreditation does not impact eligibility and is not required in order to receive customized TA from the funded national partner and CSTLTS.
This customized TA can increase the capacity of HDs and the public health system to deliver the 10 essential public health services in 3 core areas of public health and meet national public health standards. This increased capacity can lead to increased access and delivery of public health services to communities and improved health outcomes.
In collaboration with key national partners, CSTLTS will provide funding and customized support to small health departments to 1) complete projects to help improve their performance and cultivate an agency-wide culture of performance improvement, and/or 2) complete systems improvement projects to help foster, develop, and support collaboration/partnership between public health and other sectors that address the social determinants of health.
This project is intended to run for at least two years for each awardee health department. The first 3-6 months of the first year should be focused on developing a detailed project and action plan with support from the funded national partner. This time should be spent refining the project focus, ensuring that the right teams and partnerships are in place, ensuring and receiving the right level and amount of training, detailing the action plan on what activities will take place, who owns those activities, and how they will be carried out on a more granular level, and firming up any other relevant components identified as critical to success. The remainder of the 2 year period after the project and action planning period will be spent on active implementation with support and TA from funded national partner. Consideration for year 2 support is contingent upon satisfactory completion of year 1 activities.
Page 2 of 7FY20 CIO Project Plan - Customized Systems and Performance Improvement...

Cigarette smoking remains the leading cause of disease, disability and death in the US. A number of population groups are known to have a disproportionately higher prevalence of smoking, including those covered by Medicaid. The purpose of this project is for state health agencies' medicaid programs to identify adults who are state Medicaid enrollees who currently smoke cigarettes. This project seeks to determine the ability to assess smoking status among Medicaid enrollees who have a smoking related chronic disease. Finally, this project aims to retrospectively (annually) assess the smoking status of Medicaid enrollees in order to determine if these individuals have made an attempt to quit smoking or have relapsed. Once the feasibility of this is demonstrated, a potential next phase would be for state health agencies' Medicaid programs to strengthen their infrastructure and date systems to determine ways to provide covered cessation supports and services to individuals identified as current smokers who are Medicaid enrollees.
Determine if state health agencies and Medicaid programs infrastructure and data systems can identify current smoking status is collected by each state
Medicaid program on enrollees (e.g., within enrollment, EHRs, etc).
For states that do not collect smoking status in state Medicaid data, make a recommendation on how this information could be collected.
Number of state health agencies and Medicaid programs that can demonstrate how the above activity (i.e., determination of smoking status) is made within each state Medicaid programs to obtain the above information.
Provide an account of smoking indicators that are collected by state Medicaid programs.
Provide a written report on availability of smoking status information or indicators, that includes a listing of all indicators, how the information is collected and maintained by state Medicaid programs or their designee.
Increased understanding and awareness of the methods of data collection and availability of applicable datasets to determine current smoking status within state Medicaid programs.
Improved the capacity to measure current smoking status within the Medicaid enrollee population through facilitating or enhancing partnerships and collaborations among different state entities.
Improved state health agenices and Medicaid programs's infrastructure to evaluate, manage and improve standards for identifying current smokers

Obtain access to state level Medicaid claims data for up to 4-5 states (to be further determined in collaboration with CDC) that collect smoking status.
Determine if a link can be provided between smoking status indicators that have been collected by the state Medicaid program and the Medicaid claims data.
For states that do not collect smoking status in their Medicaid data, determine if this information is available within other sources (e.g., provider or insurer under contract with Medicaid)
For the states that collect smoking status within their Medicaid data, determine if other entities in the state health departments (e.g., cancer division) also identify smoking status among Medicaid enrollees.
For the states in which the data can be linked to Medicaid claims data, perform a descriptive analysis to describe the characteristics of current smokers
(e.g., demographic and health care utilization).
Determine if Medicaid enrollees who are current smokers and have a smoking related chronic condition (e.g., cancer diagnosis and treatment, as determined through analysis of Medicaid claims data) are trying to quit.
Determine if smoking status and quit attempts can be assessed in prior years in order to ascertain cessation history and relapse.
In working with the previously identified states, develop a method to ascertain smoking prevelance within the state Medicaid population using the data previously obtained and identified.
Demonstrate ability to access and analyze state level Medicaid data.
Demonstrate ability to work with state health departments focusing on chronic disease prevention (e.g., cancer).
Draft a process roadmap of how Medicaid data is obtained, analyzed, and the indicators that are used.
Develop a document showing where and how smoking status can be identified in select state level Medicaid data.
Provide a written summary of other entities in state health deparments that also collect smoking status among Medicaid enrollees.
Provide the prevalence estimates of current smoking statusstratified by Medicaid eligibility.
Provide a written summary recommendation on how smoking status could be collected for Medicaid enrollees for entities that don't already collect it.
For up to 4-5 states (determined in collaboratin with CDC), ascertain current smoking status among Medicaid enrollees who also have a cancer diagnosis, and provide a written summary of results.
For at least two states, provide a written summary quantifying smoking cessationamong this population.
Improve the capacity of existing data insfrastructure to help conduct public health monitoring by measuring the prevalence of current smoking in the previously determined states within each state's Medicaid population.
Improve knowledge of potential methods to assess current smoking status within the Medicaid enrollee population through different datasources (e.g., Medicaid data, EHR, etc).
