About this file
This document is a Commercially Useful Function (CUF) Certification Form issued by the California Correctional Health Care Services (CCHCS) for Bid Number SD24-00021. The form is designed for Small Business (SB), Microbusiness, and/or Disabled Veteran Business Enterprise (DVBE) contractors, subcontractors, or suppliers to certify their role and functionality in contract execution. The form requires businesses to demonstrate that they are responsible for distinct elements of work, actually perform or manage contract work, conduct business within their normal service functions, and add genuine value to the contract process.
The certification form requires contractors to answer six specific questions about their operational capacity and business practices, including their responsibility for contract execution, work performance, alignment with normal business services, management of contract-related materials and supplies, and their value-added contribution. The document is structured to ensure that certified businesses are not merely pass-through entities but are actively and meaningfully participating in the contract, in compliance with California Government Code and Military and Veterans Code. The form must be completed with an original signature from the certified business owner or corporate officer, who certifies under penalty of perjury that the provided information is truthful and accurate.
