Statement of Work Chancery Window Cleaning

1. SCOPE OF CONTRACT

A. The purpose of this firm fixed price purchase order is for Window Cleaning Services for real property owned or managed by the U.S. Government at Panama City, Panama.

B. The contract will be for a one-year period from the date of the contract award, with one cleanings service.

C. The price listed below shall include all labor, materials, overhead, profit, and transportation necessary to deliver the required items to the U.S. Embassy Panama located in Clayton, Bldg. 783, Panama City, Panama.

D. All prices should be in U.S. Dollars.

2. INTENT

A. Description of Project:

Contractor shall perform Window Cleaning of all windows and louvers/sunshades/fins on all exterior walls facing north, west, south and east elevation. Windows altitude ranges from one

(1) meter at ground and first floor level to twenty-six with 60/100 (26.6) meters at the Atrium east elevation. Contractor shall provide labor, materials and equipment required to accomplish the work required under this SOW. Perform final clean up.

B. Existing Condition:

The existing work area is exterior windows on Chancery Building in the NEC.

3. GENERAL REQUIREMENTS

Under this Scope of Work (SOW), the Contractor shall clean all building windows and louvers/sunshades/fins using mineral free water. Tap water shall be purified and pushed through a pole. The glass surface shall be scrubbed by a brush on the pole. Once all the impurities have been removed from the water and window surface, the contractor shall rinse the glass after scrubbing. No detergents or chemical products shall be used. Windows glass shall show an excellent streak and spot free surface. The associated exterior marble ledge surfaces shall be cleaned also. The contractor shall perform final clean up.

The Contractor shall thoroughly clean the work area daily and properly dispose of all debris and surplus materials. Equipment to remain on site at the end of the workday must be organized and secured by the contractor and will remain the sole responsibility of the contractor.

4. SCOPE OF WORK

Contractor shall provide:

1. The Contractor shall clean all building windows, and exterior louvers/sunshades/fins using mineral free water.

2. Tap water shall be purified by a reverse osmosis system and pushed through a fiberglass pole.

3. The glass surface shall be scrubbed by a brush on the pole. . The associated exterior marble ledge surfaces shall be cleaned also.

4. Once all the impurities have been removed from the water and window surface, the contractor shall rinse the glass after scrubbing. No detergents or chemical products shall be used. Windows glass shall show an excellent streak and spot free surface.

5. The Contractor shall thoroughly clean the area daily and properly dispose of all debris and surplus materials outside the Embassy grounds in accordance with local regulations.

6. The Contractor shall perform the final clean up.

5. PERIOD OF PERFORMANCE AND DELIVERABLEES

A. Period of Performance:

a. The Contractor shall be required to:

(a) Commence within five (5) working days after the date the Contractor receives the

(b) Notice to Proceed (NTP).

(c) Complete the entire work ready for use not later than thirty 30) calendar days after

(d) receiving the NTP. The time stated for completion shall include final cleanup of the

(e) premises and completion of punch list items.

B. Deliverables:

a. The Contractor shall deliver a Performance Schedule to the COR five (5) days after the award of the contract.

6. WARRANTY

The entire window cleaning works will be guaranteed during a fifteen (15) days period from the date of the final acceptance by the U.S. Government.

7. CONTRACTOR INCLUSIONS

A. Contractor will provide all labor, expendable materials, tools, equipment, and personal protective equipment (PPE) to perform a complete and safe job.

B. Contractor will provide cold drinking water for employees while on site.

C. Contractor will remove daily all trash from site and dispose of properly.

D. Contractor will provide all work trash containers and remove from site.

E. Contractor will provide a Performance Work Schedule in the form of a “bar chart” with his proposal, which will include number of working days from start to completion date for each activity.

F. Contractor will remove daily all unused materials and equipment. Equipment to remain on site at the end of the workday must be organized and secured by the contractor and will remain the sole responsibility of the contractor.

G. Contractor work will be performed Monday through Saturday from 8:00 a.m. until 5:00 p.m.

H. Contractor will provide the number of maintenance personnel to perform work with his proposal.

I. Contractor will provide an onsite quality control supervisor/foreman to direct contractor personnel.

J. Contractor will provide written proof of health insurance of all onsite contractors’ employees working.

K. Contractor and sub-contractors will provide a first aid kit for their employees on site.

L. Contractor will provide a fifteen (15) days guarantee of cleaning service that it will be free of defects.

M. USG facilities personnel will escort contractor’s employees all time.

N. Contractor will provide name and cedula numbers of all contract employees ten (10) working days prior to start of work. Provide all vehicle license plate numbers that are required to enter site-requiring clearance in advance.

8. CONTRACTOR EXCLUSIONS

A. Any additional work other than that specified.

QUALITY ASSURANCE AND SURVEILLANCE PLAN (QASP)

A. This plan provides an effective method to promote satisfactory contractor performance.

The QASP provides a method for the Contracting Officer's Representative (COR) to monitor Contractor performance, advise the Contractor of unsatisfactory performance, and notify the Contracting Officer of continued unsatisfactory performance.

The Contractor, not the Government, is responsible for management and quality control to meet the terms of the contract. The role of the Government is to monitor quality to ensure that contract standards are achieved.

Services.

Performs all Window Cleaning of all windows and louvers/sunshades/fins on all exterior walls facing north, west, south and east elevation services set forth in the scope of work.

All required services are performed and no more than one

(2) customer complaint is received per cleaning performance.

B. SURVEILLANCE. The COR will receive and document all complaints from Government personnel regarding the services provided. If appropriate, the COR will send the complaints to the Contractor for corrective action.

C. STANDARD. The performance standard is that the Government receives no more than one (1) customer complaint per month. The COR shall notify the Contracting Officer of the complaints so that the Contracting Officer may take appropriate action to enforce the inspection clause (FAR 52.212.4, Contract Terms and Conditions-Commercial Items (May

2001), if any of the services exceed the standard.

D. PROCEDURES.

1. If any Government personnel observe unacceptable services, either incomplete work or required services not being performed they should immediately contact the COR.

2. The COR will complete appropriate documentation to record the complaint.

3. If the COR determines the complaint is invalid, the COR will advise the complainant. The

COR will retain the annotated copy of the written complaint for his/her files.

4. If the COR determines the complaint is valid, the COR will inform the Contractor and give the Contractor additional time to correct the defect, if additional time is available. The

COR shall determine how much time is reasonable.

5. The COR shall, as a minimum, orally notify the Contractor of any valid complaints.

6. If the Contractor disagrees with the complaint after investigation of the site and challenges the validity of the complaint, the Contractor will notify the COR. The COR will review the matter to determine the validity of the complaint.

7. The COR will consider complaints as resolved unless notified otherwise by the complainant.

8. Repeat customer complaints are not permitted for any services. If a repeat customer complaint is received for the same deficiency during the service period, the COR will contact the Contracting Officer for appropriate action under the Inspection clause.]
