About this file

This document is a Darfur Contracting Act Certification form issued by the State of California Department of General Services (DGS) for Solicitation 25FS0032. The form applies to companies that have conducted business activities outside the United States within the previous three years and are seeking to bid on or submit a proposal for a State of California contract. Companies must certify they are either a) not a scrutinized company, or b) a scrutinized company that has obtained written permission from the Department of General Services to submit a proposal.

The form provides two options for compliance: Option #1 allows companies to certify under penalty of perjury that they are not a scrutinized company, while Option #2 is for scrutinized companies that have received explicit written permission from DGS to submit a bid or proposal. Companies with no international business activities in the past three years are not required to complete the form. The certification is made under California law and requires the company to provide its name, Federal ID Number, and an authorized signature, along with the printed name and title of the person signing the document.
