MCDOT PROJECT NO. TT0752

LOCATION OF THE WORK: This project is located in within Maricopa County, Arizona within Township 1N and Range 5E and Sections 10 and 15 in the Community of Tonto Hills.

PROPOSED WORK: This project consists of furnishing all labor, equipment, and materials necessary to perform all operations required for installing new water lines throughout the Community of Tonto Hills and removing sections of the existing water lines as shown on the plans. Contractor is advised that the existing water lines to be removed include the following materials: ACP, DIP and PVC.

This project also consists of furnishing all labor, equipment, and materials necessary to perform all operations required for sealing the existing road surfaces with chip seal where water line work will occur. The project includes traffic control, clearing and grubbing when trenching will be done outside the limits of the existing chip sealed roadway and hydro-seeding these areas.

The Contractor is advised that three Approvals to Construct (ATC)s will be submitted and approved with ADEQ prior to start of construction based on the three construction phases shown on the plans.

The Contractor will be responsible for the submittal and approval of the three Approval Of Construction (AOC) based on the ADEQ requirements. The submittal of the three AOCs must be done through the ADEQ online portal (https://www.azdeq.gov/mydeq). The Contractor will be required to establish an account with ADEQ. It is highly recommended the Contractor review the ADEQ requirements for setting an account through the online portal as this process requires signature authorities and approvals.

The Contractor is advised that testing results of the new water lines must be reviewed and approved by ADEQ prior to these new lines being placed in service. The Contractor may be able to receive an expedited review and approval of the test results from ADEQ via email submittal while the corresponding AOC is being reviewed. However, there is no guarantee ADEQ will grant such approval. The Contractor shall assume that an AOC must be approved before the new water lines are active and in service.

The Contractor will be required to perform exploratory trenching to locate the existing water line segments to be removed. Once the locations of the water lines are identified and the locations of the new water lines to be installed are marked 5 feet away, the Contractor shall backfill and compact the exploratory trench with native material. Backfilling shall adhere to MAG requirements. The Contractor may use steel plates to cover locations where the new water line segments will tap the existing water lines. Exploratory trenching shall be paid for under Item

350.00990 Exploratory Trenching, Backfill and Compact and shall be paid for per linear foot.

The Contractor will be required to coordinate with Tonto Hills Domestic Water Improvement District (THDWID) to be granted access to submit the AOC on behalf of THDWID. All

TT0752 Tonto Hills Water Line Improvements 4 coordination with THDWID shall be the responsibility of the Contractor. The contact at THDWID is Mr. Jeff Frazey: jjfrazey@gmail.com.

In addition, all coordination with THDWID with regards to testing of the new water lines and service connection shall be the responsibility of the Contractor. The Contractor shall contact THDWID at least 72 hours in advance of any planned water service shutdown or service interruptions. Shutdowns shall not be longer than 4 hours.

The Contractor may develop a different phasing plan; however, the plan must be approved by the Engineer and Tonto Water Improvement District, and the Contractor shall be responsible to submit and get the approval for a new ATC from ADEQ. Construction duration may not be extended and the project schedule shall be maintained should the Contractor submit a new ATC.
