About this file
This document is a Certification of Drug-Free Workplace form (Attachment J) for the Florida Department of Management Services' Solicitation No. 25-84131512-ITB for Electronic Data Processing Equipment Insurance. The form is designed to be submitted by bidders as part of their proposal, with a preference given to vendors who have implemented a comprehensive drug-free workplace program. The program requirements include publishing employee statements about controlled substances, informing employees about drug abuse dangers, providing policy statements to employees, requiring notification of drug-related convictions, and maintaining ongoing drug-free workplace efforts.
The form outlines six specific requirements for a drug-free workplace program, including publishing prohibitions on controlled substances, informing employees about drug abuse risks, distributing policy statements, mandating conviction notifications, imposing sanctions on employees with drug violations, and making good faith efforts to maintain a drug-free workplace. By signing this certification, the bidder attests to full compliance with these requirements, understanding that false statements are punishable under law. This document serves as a supplementary qualification for bidders in the Electronic Data Processing Equipment Insurance solicitation, potentially providing a competitive advantage in the event of equally ranked bids.
