About this file
This is a bid form attachment for the California WIC Infant Formula Rebate IFB 25-10009, specifically for milk-based infant formula. The form requires manufacturers to provide detailed product information including product name, UPC code, unit size, reconstituted ounce per unit, and lowest national wholesale price per unit for full truckload delivery across three physical forms: powdered, liquid concentrate, and ready-to-feed formula. The form specifies that no minimum or maximum number of infant participants is guaranteed, and no assurances are provided regarding quantity, type, or physical forms that will be issued under the agreement.
The pricing structure requires bidders to submit rebate amounts per unit, with calculations automatically performed to determine net costs and percent rebates. The form includes detailed monthly usage estimates based on infant age categories (0-3 months, 4-5 months, and 6-11 months) for both fully formula-fed and partially breastfed infants. The estimated monthly volume shows approximately 93 million reconstituted ounces for powdered formula, 370,465 for liquid concentrate, and 4,960 for ready-to-feed formula, based on six months of the most recent data available.
