The certifications cover workers' compensation insurance requirements (LC §3700), prevailing wage requirements with a $200 per day penalty for non-compliance, work hour limitations of 8 hours per day and 40 hours per week with a $25 per day penalty for violations (LC §1777.5), apprenticeship requirements including proper registration and wage standards (LC §1777.5), and recordkeeping requirements (LC §1776). The form requires a contractor's signature and date to acknowledge understanding and compliance with these labor code provisions.
