About this file

This document is an addendum to an Invitation for Bid (IFB) issued by the California Department of State Hospitals - Coalinga for Foundations, Utilities, Data Connections, Plumbing, Fencing, and related Install Services at their facility. The project involves grading, concrete foundations, utilities connections, asphalt/concrete work, and demolition and installation of an additional pad. Bidders are asked to provide labor rates and estimated hours for the required work. The IFB addresses several bidder questions regarding fencing, shade structures, equipment storage, water and electricity access, and the need to remove and replace the existing concrete pad.

The bid rate sheet provides line items for the various project components, with bidders required to provide unit prices. The scope of work indicates that the contractor will be responsible for inspecting the current infrastructure and utilities, and providing a report to the Department on any work needed to meet regulatory requirements. The Department will then determine if the existing pad and infrastructure need to be replaced. No shade structure installation is required. Storage containers may be allowed on-site depending on the size and power needs. Water and electricity access is available near the job site.

Please see the following questions regarding the IFB, as previously addressed:

1. Sheet C1.1, Keynote 3, portion of the chain link fence will need to be removed. Can the removed section remain open until the new section is installed? The removed section can remain open when the work is being conducted during the day;

However, after-hours temporary fencing will be needed. R&R is the main area for all patients leaving and entering the facility and cannot be unsecured.

2. Sheet C2.1, General note 2, Describes the installation of a shade structure. Is there a shade structure to be installed, if so, where can the information be found? No shade structure installation will be required.

3. Will we be able to place a storage container on site to house all tools and materials so that we don't have to check them in each day at the Sally Port? Storage containers have been approved in the past, depending on size. If you require power to your storage container, DSH may require a permit.

4. Is there a source of water nearby where we can use for the jobsite? There are 3 options for water within the vicinity of the job. Currently there are 2 hose bibs approximately 200 feet from the site.

5. Is there a location where electricity can be utilized if needed? The existing concrete pad has an electrical panel with a standard 120V electrical outlet readily available for use.

6. Is there a laydown area within the security fence that can be used for storage of excess dirt, asphalt, concrete until it can all be removed off site? This could be an option depending on how much material is required and how long storage is needed.

24511 W. Jayne Avenue

7. Will we be able to leave earthwork equipment (tractors) on site within the jobsite fenced area instead of (exiting the sally port) storing it outside of the secured fenced area? Storage will only be allowed outside the secured fenced area.

8. How early can construction start? If no escort is required, construction can begin at 7:00AM. If an escort is required, construction can begin at 7:30AM.

9. Are we to propose on the removal of the existing pad and install a new pad? Yes. For further information, please refer to IFB Attachment 5 (Bidder Rate Sheet), IFB Exhibit A.7 (Contractor Responsibilities, Item G) and IFB Exhibit B. 5 (Budget Detail, Item E).
