About this file
This is a pricing schedule (Schedule B) for Request for Proposal No. 210000001676 issued by the State of Michigan for the Women, Infants, and Children (WIC) Infant Formula Rebate Program. Bidders must submit pricing for milk-based infant formula in three physical forms: liquid concentrate, powder, and ready-to-feed formats. The pricing schedule requires contractors to enter manufacturer name, product brand name, unit size in ounces, reconstituted ounces per unit, wholesale price per unit, and rebate per unit for each formula type. The bid evaluation worksheet projects monthly consumption based on historical data from March 1, 2020 through February 28, 2021, with calculations spanning multiple age groups including infants from birth through eleven months, with separate tracking for IBP (Infant Based Products) and IFF (Infant Formula Formulations) categories.
All pricing must be submitted in a modifiable format such as Microsoft Excel or Word, with an optional non-modifiable PDF version permitted. Wholesale prices and rebates per unit must be submitted with three decimal places, and all costs must be included in the pricing, such as shipping, handling, per-piece pricing, and palletizing charges. The bid calculation sheet automatically computes net costs by subtracting the rebate per unit from the wholesale price per unit and multiplying by projected monthly unit volumes. Failure to complete the pricing schedule as requested may result in proposal disqualification. By submitting a proposal, contractors certify that prices were developed independently without consultation or agreement with other contractors.

SCHEDULE B - PRICING
Request For Proposal No. 210000001676
Women, Infants, and Children (WIC) Infant Formula Rebate Program
1. The Contractor must provide a pricing schedule for the proposed Contract Activities using Schedule B, Pricing. The pricing schedule should be submitted in a modifiable format (e.g. Microsoft Word or Excel); however, you may also submit an additional pricing schedule in a non-modifiable format (e.g., PDF). Failure to complete the pricing schedule as requested may result in disqualification of your proposal.
2. There are three tabs to Schedule B Pricing: 1) Instructions, 2) Bid Sheet and 3) Bid Calculation Sheet.
3. Under the “Bid Sheet” tab, Bidders must enter the following information: manufacturer's name, product name, powdered unit size, reconstituted ounces per unit, wholesale price per unit, refer to Schedule A, Section 4.1 Price Term (on the date of the bid opening), and rebate per unit into the chart. Data entry fields are in bold in green sections. Use the tab key to move between fields. Note that wholesale prices and rebates per unit must be submitted with 3 decimals. Calculations will be done automatically within the spreadsheet in the “Bid Calculation Sheet” tab.
4. Price proposals must include all costs, including but not limited to, any one-time or set-up charges, fees, and potential costs that Contractor may charge the State (e.g., shipping and handling, per piece pricing, and palletizing).
5. Participation and infant formula data does not necessarily reflect actual issuance and redemption that will occur under the contract resulting from this RFP.
6. By submitting its proposal, the Contractor certifies that the prices were arrived at independently, and without consultation, communication, or agreement with any other Contractor.
