About this file
This document provides answers to vendor questions for an amendment to a solicitation seeking pharmacy services and related services for the Armed Forces Retirement Home in Washington, DC. The amendment maintains the proposal due date of January 7, 2020 at 2:00 PM ET. It clarifies that drug and medication pass reviews are required monthly according to state, federal, and AFRH standards. While a formulary is not currently required, the emergency dispensing system and two medication carts per floor must be provided by the contractor and audited every other month. Prescriptions will continue to be filled for a seven-day supply. The contractor must submit documentation showing staff meet health requirements but no onsite staff are needed. This amendment provides additional details for an existing pharmacy services contract currently held by Millennium Pharmacy.

Question 1 - Is this a new requirement, or is there an incumbent currently providing these services? If there is an incumbent currently providing these services, who is the incumbent?
Answer 1 - This is an existing contract with service provided by Millennium Pharmacy.
B. 4.6 Drug Reviews/Medication Pass Reviews. The contractor shall perform periodic drug reviews and medication pass reviews at the Washington, D.C. facility. Drug reviews and medication pass reviews shall be conducted monthly at the beginning of contract performance. Drug reviews and medication pass reviews shall be in accordance with state, federal and AFRH requirements (monthly). Reports and any additional documentation as required should be provided upon completion of reviews or as requested by AFRH.
Q2 - Will the contractor provided medication pass review be conducted in addition to the review that is standardly conducted by the facility staff?
A2 - Yes, the expectation is quarterly, and upon request usually during survey window.
C. 4.8 Formulary. The contractor shall have a current formulary targeted specifically to the geriatric resident.
Q3 - Is the requested formulary representative of a list of drugs that are available through the contractor¿s sources specifically? If so, is this list of drugs in addition to those prescribed by the patient¿s physician, as well as those which might be in stock at the facility?
A3 - No, not at this time.
D. 4.9 Equipment Needs. The contractor shall supply each facility with adequate medication carts, as necessary (estimated two (2) medication carts and one fax machine or computerized system per floor) to transmit prescription needs. The
AFRH will determine the number of carts required based on the packaging system proposed by the

Q4 - Does the facility have existing medication carts?
A4 - Yes, provided by the contractor.
Q5 - Is there a specific type/brand cart necessary for this requirement?
A5 - No specific brand of cart is needed. The requirement is an internal lock for the narcotic lock box inside the cart and an external lock for the cart itself. This double lock system is mandatory for medication compliance.
Q6 - Does the facility have a dedicated and secure location in which the cart and fax will be located?
A6 - The emergency dispensing unit is located behind in the nursing station behind a locked door. There is a dedicated area for the medication cart but it is not behind a locked door. The practice is ok as long as the medication cart is locked.
Q7 - Will prescriptions be written for a 30-day supply when written? Is it the expectation of the AFRH that the prescriptions are filled and dispensed as written by a local retail pharmacy?
A7 - AFRH-W receives a seven-day supply of medications. This has been the practice to avoid receiving too much of a high cost medication that a resident may not use.
E. 4.10 Emergency Dispensing System. The contractor shall provide an emergency /back-up system (preferably an electronic dispense system). The contractor shall audit the electronic dispensing system every other month to ensure compliance.
Q8 - Is there an existing emergency dispensing system currently on the premises?
A8 - Yes, provided by the contractor.
Q9 - Would the AFRH be opposed to having prescriptions filled and dispensed by a retail pharmacy versus a machine?
A9 - Currently, AFRH-W receives packaged medications from our pharmacy. The emergency medication are taken from the electronic

F. 4.13 Pre-Authorizations. All pre-authorization forms must be completed by the contractor and not AFRH.
Q10 - What is an example of when it would become necessary for a prior authorization?
A10 - Currently our physicians are not involved in prior authorizations. Prior authorizations are expected to be handled by contract pharmacy. The physicians are involved only when a signature is needed by a specific insurance company.
Q11 - Is it anticipated that the below listed professionals will perform services at the facility? If so, what is the anticipated frequency that each title will staff the facility?
¿ Pharmacist(s) licensed in the State of contract performance (Washington, DC)
¿ Registered Nurse licensed in the State of contract performance (Washington, DC)
¿ Minimum of an RPH or Doctorate RPH degree from a nationally recognized college or university covering such degree in Pharmacy.
¿ The Registered Nurse must have an RN/BS degree.
A current unrestricted pharmacy license to practice in the state/district of the United
States where the services are being performed.
The contractor shall be responsible for complying with all state, federal, local and AFRH requirements.
A11 - There are no contract employees working out of our facility.
G. 9. HEALTH REQUIREMENT. Contractor personnel performing services at AFRH under this contract shall be free of communicable disease. A purified protein derivative test for tuberculosis (PPD) shall be required. If the PPD is negative, a
