Appendix B: Past Performance Questionnaire Instructions The Medicaid and CHIP Payment and Access Commission (MACPAC) appreciates your time and thoughtfulness in completing this past performance questionnaire. The information included in this questionnaire will be used to evaluate the past performance of offerors responding to solicitation no. 202601, “MACPAC Analysis and Management of Administrative Data.” Please note that the questionnaire will only be considered complete if all performance ratings at or below “Satisfactory” include comments to support the rating.

The questionnaire requests information on the technical performance and quality, management approach, and schedule of the offeror. Please complete the questionnaire as indicated on the form. All information submitted as part of this questionnaire will be kept confidential and not disclosed outside of MACPAC.

Performance Ratings The following rating and definitions (Table 1) will be used by the evaluator to rate the performance of the offeror in Table 2. For additional information on ratings, please see Table 42-1 found at FAR 42.1503(h)(4).
