About this file

This is a Prime Contractor Response Qualification Form issued by the Minnesota Department of Natural Resources for Solicitation #2026-570, which concerns the Forest Lake #2 Public Water Access (PWA) Parking Lot Reclaim and Repave project in Washington County. The project involves full surface milling and overlaying of approximately 20,000 square feet of parking lot, including reclaiming the existing 2 inches of bituminous and aggregate base to a 6-inch depth, grading and compacting reclaimed material, and paving with 2 inches of bituminous base and 1.5 inches of bituminous wear per Minnesota DOT specifications. The work also includes topsoil preparation, seeding, and mulching of pavement edge, with parking lot grades maintained to meet ADA guidelines and handicap stall slopes not exceeding 2 percent. All responses must be submitted electronically through the Swift Portal, and interested contractors may contact Project Supervisor Jason Ko at 651-677-8483 to schedule a site visit. The contractor assistance help desk is available at 651-201-8100, Option 1, for registration and response completion support.

The qualification form requires that all prime contractor responders have substantially completed at least two written and verifiable contracts since January 1, 2021, for reclaiming, repaving, and striping work with project sizes of 8,000 square feet or greater and contract values of $15,000 or greater. Additionally, responding companies must certify that they have not been found in default or had a contract terminated for cause within the last 36 months and have not received any willful or repeated OSHA safety citations for which a final order has been issued within the last 36 months; responses answering "yes" to either of these conditions will result in automatic rejection. Unit pricing must include all costs such as labor, materials, tools, transportation, equipment, and applicable taxes, and the award will be made to the lowest responsible vendor meeting all specifications and terms and conditions of the solicitation.
