About this file

This is an Affidavit of Eligibility Form for participation in the Buy Local or Veteran Small Business Program, applicable to a Ferric Sulfate Supply and Delivery solicitation in Florida. The form serves as a certification document that businesses must submit with their bid responses for solicitations that include Buy Local or Veteran preference set-asides. Bidders must select and certify their eligibility under one of two program categories: the Local Business (Buy Local) program or the Veteran Small Business program. The form requires completion of business information fields including business type, business name, and an authorized signature affirming that the business meets the stated requirements.

For Local Business (Buy Local) certification, businesses must be headquartered, have a manufacturing facility, or maintain a principal office within Orange, Osceola, Lake, Polk, or Seminole County for at least six months prior to solicitation issuance, or have the owner/principal maintain primary residence within these jurisdictional boundaries. Applicants must provide a physical business address (excluding post office boxes), a valid and current local business license tax receipt issued by a local government within the specified counties, and any additional verification information requested by the district. Veteran Small Business applicants must demonstrate that the business is owned and controlled by veterans as defined in Section 502 of the Veterans Benefits, Health Care, and Information Technology Act of 2006, is domiciled in Florida, and is listed in the federal Database of Veteran-owned Businesses, with required proof of veteran ownership and small business status.
