SOW: BurnAlong

FY2025

STATEMENT OF WORK

(SOW)

I. SCOPE:

A. BACKGROUND

BurnAlong is a virtual wellness platform that delivers instant access via licenses to over 5,000 members in over 63 countries for health/wellness and social interaction. Contractor/Contract provides an application through mobile phone, tablet, computer, and/or SmartTV to access over 50,000 classes/100 programs/10,000 instructors/70 categories. BurnAlong connects employees with varied content, wellness challenges, social motivation, and immersive events.

BurnAlong allows employees to easily interact with wellness resources on demand through their CMS laptops with coworkers, family and friends from anywhere, any place, and anytime. BurnAlong offers a wide variety of categories, including physical fitness, emotional support, mental health, nutrition, medical well-being, and financial planning. Additionally, BurnAlong provides fitness workouts for various levels of fitness and ability. BurnAlong collaborates with other key CMS programs such as New Employee Orientation (NEO), managerial trainings, Wellness Wednesdays, and Public Service Recognition Week. BurnAlong would allow employees to improve work‐life balance according to their needs and interests.

This procurement is a one (1) year contract, with one (1) year option.

Privacy Policy

BurnAlong follows strict security guidelines based on ISO27K as well as HIPAA guidelines. The data is stored and transmitted encrypted from client to devices. BurnAlong does not share, sell, or lease our data. Any individual can request that their account and data be irreversibly deleted.

Computer Access– employees can access classes via computer and can access applications via FaceBook, LinkedIn, and Google. If employees choose this process, the contractor will obtain information from these sites as follows:

· Facebook. If employees log into BurnAlong using Facebook, they must enter their Facebook email address and password. The contractor requests the employee to grant permission to:

· Access and collect Facebook basic information (this includes name, profile picture, user IDs, list of friends, age range, email address, link, location, time zone and any other profile information the employee has set to public).

· If employees allow access to this information, BurnAlong will have access to this information even if the employee has chosen not to make that information public.

· LinkedIn. If employees log into BurnAlong using LinkedIn Account, they must enter their LinkedIn email address and password. By logging in through LinkedIn, they are granting BurnAlong access to their profile information (this includes your name, location, profile picture, and contact information).

· Google. If employees log into BurnAlong using Facebook, employees must enter their Google account email address and password. By logging in through their Google account, they are granting BurnAlong access to their profile information (this includes your name, profile picture, gender, user IDs, date of birth, email address, location, and any other information you have set to public).

Mobile - When employees provide their mobile telephone number and if they choose to opt in to receive system messages and/or motivational messages, they agree that BurnAlong may send them text messages (including SMS and MMS) to that mobile telephone number. They can adjust the frequency of messages in their profile settings on go.BurnAlong.com. Options for motivational messages that may be available include daily, weekly or monthly. Options for class reminders may include 24-hour notice, 5-hour notice or 5-minute notice.

BurnAlong will never charge employees for the text messages they receive; however, they may see message and data rate charges from their mobile provider. Employees are responsible for the payments to their mobile providers of any message, data and other charges related to text messages, including confirmation texts received and responses thereto. Texts may be sent through an automatic telephone dialing system. Consent to receive mobile messages is not required as a condition to using Go.BurnAlong.com or its apps.

B. PURPOSE

The purpose of providing the BurnAlong platform to CMS employees is to support employees with instant access to physical and emotional tools. The BurnAlong platform serves as a digital and local health solution that provides immersive online classes, programming, and personalized experiences that drive engagement and build culture to maximize human potential. This tool enhances and increases CMS employees’ overall wellness and morale and reaches all CMS employees nationwide, particularly regional office employees without an onsite fitness center.

This Agreement, together with any Exhibits attached hereto which are hereby incorporated by reference (the “Agreement”) is effective between Contractor/Contract and, CMS (“Client”) and issued in accordance with the included terms and conditions. This Agreement, including any appendices, schedules, and/or attachments, documents the understanding between Contractor/Contract and Client with respect to providing the deliverables defined below.

II. REQUIREMENTS:

A. GENERAL REQUIREMENTS

The Contractor shall provide:

· Flat fee a month, includes 3,000 licenses for up to 3,000 employees with sub-accounts for up to 4 additional family members.

Services will start on the date the contract is signed and run for an initial period of twelve (12) months. Each contract year shall stand alone and is subject to the government exercising the option years. The government is only agreeing to one (1) year of service. CMS reserves the right to not execute any or all option years for a total of two years.

B. SCHEDULE OF DELIVERABLES

*5% Discount if paid in full at time of signing. BurnAlong must be notified within 5 days of signing of intention to pay in full.

Terms

Services provided under this Agreement will terminate on the expiration date of the Term or the date terminated by either party as provided in the Corporate Subscription Agreement. The Contract with contiguous dates will ensure no gap in service.

III. QUALITY ASSURANCE:

The Federal Government, through the Contracting Officers’ Technical Representative, will review and approve all work products and deliverables submitted by the contractor. Monthly project management meetings shall be held between the contractor and the Contracting Officers’ Technical Representative. The monthly meetings will be used to review the status of the contract, upcoming events, outstanding issues, and fiscal concerns.

Form and Content – The contractor shall provide timely deliverables that are clearly written, well organized, and correct in grammar and spelling, and specific to the contract requirements and CMS’ needs.

Timeliness – The contractor shall meet established timelines. The contractor shall submit all deliverables to CMS to ensure receipt by or before the specified due dates. CMS will evaluate the contractor’s performance using measures such as the following:

· Performance of tasks in accordance with the schedule set forth in this contract.
