About this file

This document is an Invitation to Bid (ITB) Bid Schedule (File No. 2025-1600-0247) for a comprehensive cleaning and maintenance contract, likely for a Public Health Center (PHC) in Alaska. The bid covers a five-year contract for thrice-weekly, weekly, and twice-yearly cleaning and maintenance services, with services to be performed on a Monday, Wednesday, and Friday schedule. The bid is due on Wednesday, April 16, 2025, at 2:30 p.m. Alaska time, with a strict total project cost limitation of $160,000. The services include detailed cleaning tasks such as garbage removal, disinfection of exam rooms and surfaces, glass and mirror cleaning, bathroom sanitation, floor maintenance, entry mat management, and year-round ground maintenance including seasonal lawn care and snow removal.

The bid schedule is structured to allow vendors to input individual service rates, which will automatically calculate monthly, annual, and five-year total costs across three service categories. Bidders are instructed to fill only in green-highlighted cells, with the remaining cells containing locked formulas for automatic calculation. The pricing structure requires vendors to provide rates for specific deliverables in each service category, including thrice-weekly nightly services (15 specific tasks), weekly services (4 specific tasks), and twice-yearly services (3 specific tasks). The document emphasizes precise cleaning standards, including the use of tuberculocidal/virucidal/fungicidal/bactericidal cleaning products and specific maintenance requirements for different areas and surfaces within the facility.

Sheet1 Attachment 2 - Bid Schedule

ITB 2025-1600-0247

The purpose of the cost proposal format below is to allow offerors to submit pricing in a consistent manner that the State can evaluate and score. In accordance with ITB Section 1.02 - Budget, total project cost more than $160,000 will be considered non-responsive.

Instructions for filling in costs: Bidders are to fill in only the cells highlighted green. The remaining relevant cells contain formulas and will total automatically. All cells apart from those bidders are to fill in are locked and bidders are not to attempt to overcome this.

For "Thrice-Weekly Nightly Services," determine and fill in the individual cost of each nightly service (green cells B4-B18). Column C will automatically fill in the totals for what each deliverable will cost to perform for four weeks (one month). Columns D through H will each automatically fill in the cost of twelve months. These represent Years 1-5 individually. The cells in Column I will formulate the total for all five years for each deliverable. Cell I19 will total the cost of all thrice-weekly deliverables for five years.

Under "Weekly Services," fill in the rates, in cells B21-B24, to perform the deliverables. The remainder of the row will automatically fill in as it did for Weekly Services, in this case showing the monthly rate (service performed four times), each individual year's rate, and the total for all five years. Cell I25 will total the cost for all weekly services for all five years.

Under "Twice-Yearly Services, determine and fill in the individual cost of each Twice-Yearly Service (cells B27-B29). Columns D-H will automatically fill in the totals for what each deliverable will cost to perform twice in Years 1-5. Column I will formulate the total for all five years for each deliverable. Cell I30 will total the cost of all twice-yearly deliverables for five years.

Cell I31 will automatically fill in the final total.

Below the table, fill in vendor name and contact information (including "Submitted by").
