About this file
This document is an Application for Personnel and Visitor Passes for the Air Force/Air National Guard, specifically for contractors and visitors seeking access to the 167th Airlift Wing in Martinsburg, West Virginia. The form requires comprehensive personal information including full name, social security number, date of birth, citizenship status, and vehicle details. Contractors and visitors must attach a legible copy of their driver's license or state ID, and non-US citizens must provide current immigration documents. The form includes sections for contractor certification, contracting office verification, and security forces pass and ID processing, with a mandatory disclosure that failure to provide requested information will result in access privileges being refused or withdrawn.
The related federal contract opportunity is a small business set-aside solicitation (W50S8V-25-B-A011) for two separate projects at the 167th Airlift Wing: a gutter repair project for Building 307 and an interior renovation of Building 309 for operations and training. The project is classified under NAICS 238160 with a size standard of $19.0M, and is expected to be valued between $25,000 and $100,000. Davis-Bacon Wage Rates apply, and while funds are not currently available, the government anticipates funding prior to award. The contract will be awarded to the most advantageous responsible bidder, considering only price and price-related factors.
