About this file
This document is Amendment 3 to a Request for Qualifications and Quotations (RFQQ) issued by the Washington State Health Care Authority (HCA) for Dental Authorization Review Services. The amendment adds a list of interested subcontractors, including CVF Dental Consulting, LLC, led by Dr. Cynthia V. Feleppa, who has extensive experience in dental authorization adjudication, quality assurance, and advocacy for clients accessing state services. The original RFQQ sought bids from Washington State licensed dentists to conduct dental authorization adjudications for the Apple Health fee-for-service dental program, provide expert testimony at dental hearings, and assist other HCA units as a subject matter expert.
The contract opportunity covers the state of Washington, with Thurston County as the primary jurisdiction. Pricing terms, set-asides, funding requirements, and other salient information were not provided in the amendment. The amendment indicates that all other terms, conditions, and specifications of the original RFQQ remain unchanged.

1. Dr. Feleppa served 22 years as a U.S. Army dental officer and transitioned to civilian life in October 2023. Through the military Career Skill-bridge Program (CSP), she completed a dental consultant internship with the Washington State Health Care Authority and United Concordia Dental. Her most significant military roles include serving as Deputy Director of the Advanced Education in General Dentistry – 12 month program for four years and as Deputy Director for a dental team comprised of sixteen service members deployed to Kuwait for ten months. She gained working knowledge of the judicial domain during her time as a Quality Assurance Investigation Officer, 15-6 Investigation Officer, Board of Inquiry President (Alternate), and Record Audit Officer. In addition, as a special needs child advocate, she has extensive experience navigating Apple Health, the Developmental Disabilities Administration (DDA), and successfully securing special needs state services available in Washington. She is comfortable researching Washington Administrative Codes and intimately understands their significant impact on clients contending to access state services.
2. During Dr. Feleppa’s 4-month internship with HCA, she conducted dental authorization adjudications for the Apple Health fee for service dental program. She utilized ProviderOne (P1), NEA, MS Teams, MS Excel, MS Word, MS PPT and MS Outlook to accomplish the program’s mission and expectations. She reviewed supporting clinical documentation and made medical necessity determinations on prior authorizations (PA). She understands the HCA adjudication process and the clinical documentation details that meet the criteria for Approve, Deny, Reject or Pend. Dr. Feleppa based her determinations on HCA policies, guidance and WAC regulations. When appropriate she collaborated with the dental program staff and other consultants on complicated cases and dental clarifications (DC). She participated in consultations with HCA attorneys and other dental consultants prior to dental hearings discussing the merits of clinical justifications used to determine client medical necessity eligibility. One of her strengths included work she accomplished for the Audit/Quality http://www.hca.wa.gov/
Management Team which included chart reviews and provider audits. Dr. Feleppa enjoyed her time working with the HCA and seeks to resume her work with the dental program in support of the HCA mission.
