About this file
This document is a Required Affidavit for Bidders, Offerors, and Contractors issued by the Commonwealth of Kentucky, specifically referencing Kentucky Revised Statutes (KRS) 45A.110 and 45A.115. The affidavit is a mandatory legal document that must be completed by any bidder, offeror, or contractor seeking to be awarded or renew a contract with the Commonwealth. The form requires an authorized representative to swear and affirm under penalty of perjury that the contractor has not knowingly violated campaign finance laws of Kentucky and that the contract award will not violate any campaign finance provisions.
The affidavit is valid for one year from the date of execution and includes spaces for the bidder's signature, printed name, title, date, company name, address, and optional vendor code. A notary public must witness and validate the document by completing the bottom section, which includes the date of notarization, state and county of execution, notary signature, and notary commission expiration date. The Commonwealth specifically retains the right to request an updated affidavit at any time, and the document defines "knowingly" as being aware or should have been aware of any potential violation.
