About this file

This document contains a past performance questionnaire for federal contractors. The questionnaire is to be completed by references for a contractor that submitted a proposal to provide technical assistance and support services to the Centers for Disease Control and Prevention under the Safe Healthcare, Epidemiology, and Prevention Research Development Program. It requests ratings on a scale of unsatisfactory to exceptional for the contractor's performance on past similar contracts, including factors such as knowledge and skills, meeting requirements, communication, resources, responsiveness, subcontractor management, deadlines, quality, and reporting. The reference is also asked to provide details on any defaults, disputes, strengths, weaknesses, and whether they would choose the contractor again. The questionnaire aims to accurately evaluate the contractor's past and present performance when assessing the current proposal.
