ATTACHMENT 5 Sample Task Order 2

STATEMENT OF WORK

Domain 2: Stakeholder Engagement, Assessment and Analysis

Background and Need

Worksites have a great opportunity to help workers establish healthy habits during the workday that can help them prevent health problems such as diabetes, depression, and heart disease. Worksite Wellness is influenced by workplace culture and the physical features of the worksite. Well-designed worksites keep employees safe and encourage physical activity during the workday. Places to walk and exercise, availability of nutritious foods, and environments that allow for stress relief and mental breaks help support a healthy workforce.

The purpose of this sample task order is to assist CDC in developing a stakeholder informed framework for assessing acceptability, feasibility, and the health and economic impact of implementing comprehensive worksite wellness programs for small (<20 employees), medium (21-100 employees), and large (100+ employees) organizations.

Objectives

The purpose of this task order is to develop materials to support the implementation of comprehensive worksite wellness programs for small, medium and large organizations.

Scope of Work

The contractor shall provide all labor, expertise, materials, supplies, equipment, and facilities to perform all services required to carry out the work under this task order. The Offeror shall describe their capacity and approach to conducting the work listed in this sample task order.

Task 1: Start Up Meeting

· The Contractor shall arrange and conduct an initial meeting (either in person or by teleconference) with the CDC Contracting Officer’s Representative (COR) and other relevant staff within two weeks of the effective date of this task order. The purpose of the meeting will be to review and clarify the scope of work and delivery schedule for tasks, to delineate roles and responsibilities, and to establish communication protocols. Each task and deliverable will be reviewed.

· The Contractor shall submit an agenda for the above meeting within one week of the effective date of this task order.

Task 2: Maintain Communication with the COR

· The Contractor shall coordinate with the CDC COR. The Contractor shall maintain regular communications with the COR and CDC staff involved with the project. The chief means of communication shall be via conference calls that will be scheduled every month throughout the project period required to complete the project. Additional calls will be scheduled as needed. The Contractor shall provide an agenda and meeting minutes for each call. All conference calls shall be scheduled and administered by the Contractor. The Contractor shall respond to all emails and telephone communications from the COR within 2-3 working days.

Task 3: Quarterly Progress Reports

· On the tenth (10th) day following the 3-month period being reported, the contractor shall prepare and submit a quarterly progress report containing the current status of work and a financial status report. The report shall contain, at a minimum, the current progress on each of the requirements set forth below, the financial status of the project, problems encountered during the month, solutions implemented, planned activity for the upcoming month and any other information mutually agreed upon by the contractor and the CDC COR.

Task 4: Identification of Evidence-based and Promising Worksite Wellness Approaches

The Offeror shall lay out a plan that addresses each task below. New data collections are assumed not to be necessary. Thus, the plan for each question should note what data sources will be used to answer each question, what type of analyses (e.g. literature review) will be conducted, and how conclusions will be drawn.

· Identify, through literature review and other methods, successful worksite wellness programs currently in operation in the US

· Determine and define the scope of services offered by worksite wellness programs (i.e., clinical, counseling, on-site, use of incentives, reach into the community),

· Identify approaches with documented evidence of effectiveness as well as those approaches that currently appear to be promising, but lack strong evidence,

· For each evidence-based and promising approach, describe the health and economic impacts on employees as well as other individuals associated (e.g., families, community members) and the economic benefit or cost to small, medium and large employers,

· Listing by business size (i.e., small, medium and large) the barriers faced when implementing a worksite wellness program, evidence-based approaches, and promising approaches, and identifying viable solutions by business size that have been successfully implemented.

Task 5: Stakeholder Engagement, Assessment and Analysis

The Contractor shall perform the following tasks:

· Engage stakeholders of small, medium, and large businesses in a discussion of strategies for promoting the adoption of the evidence-based and promising approaches identified

· Identify and recruit up to nine (9) stakeholders each from small, medium, and large businesses (N=27) to provide their recommendations on how CDC can promote the adoption of worksite wellness programs

· Plan and facilitate three separate meetings (one for stakeholders from small businesses, one for stakeholders from medium businesses, and one for stakeholders from large businesses)

· Develop materials based on Task 4 for stakeholders to review prior to the meeting and utilize during the meeting to develop recommendations

· Draft an agenda to be approved by CDC

· Utilize transcription services to provide a transcript of each meeting

· Provide CDC with a transcript of the meetings

Task 6: Action Report for CDC

The Contractor shall perform the following tasks:

· Develop an action report for CDC on how to promote worksite wellness programs based on all the tasks conducted within this task order. There shall be a total of 3 versions of the action report submitted for CDC approval: preliminary, final draft, and final. The Contractor shall present a final report of the project to the CDC COR within four (4) weeks after receiving comments and recommendations on the draft report. The Contractor shall submit the final report incorporating CDC’s comments and recommendations. The submission shall be in both paper and electronic format.

· Obtain stakeholder feedback on the final draft report at the same time it submits the final draft report to CDC for review.

· Submit electronically the final report to the COR. The finalized report is to be “camera ready”, meaning that the report is accurate, complete, and suitable for preparation of negatives. The camera-ready copy shall be printed on one side of the page only. Pages shall be numbered and formatted according to CDC’s convention (e.g., lower-case Roman numerals for front matter, Arabic numbers for body of the report); pages shall have a 1” margin on all sides. Endnote or other reference manager software shall be used to compile reference lists and bibliography. Citations should be “grabbed” electronically by the software from PubMed or another source to render them as accurate as possible. The report shall be submitted via email in Microsoft Word format to enable use in Web preparation. Appendices shall be submitted via email. Electronic files for charts, etc., prepared in another program (e.g., Excel) shall also be submitted. Tables and charts must be delinked from underlying databases.
