About this file

This is a contractor information sheet template used by CBP's Office of Professional Responsibility for visitor access requests. The form requires detailed personal information from contractors including name, date of birth, citizenship status, place of birth, occupation, address, and identification documents (passport, visa, driver's license, Social Security number). The template supports up to 84 visitor entries.

The form includes specific submission requirements, stating that requests must be submitted 72 hours prior to arrival via email or fax. Important procedural notes specify that visitors must present U.S. Government-issued photo ID, check in/out at security, and be escorted at all times with a ratio of one escort per four visitors (or one-to-one depending on vetting status). This is a standard form template and does not contain specific contract requirements or solicitation details.
