About this file
This contract terms and conditions document outlines an annual procurement for defibrinated blood products by the Maine Department of Health and Human Services, Health and Environmental Testing Laboratory. The contract covers three primary products: defibrinated rabbit blood (50 mL), defibrinated sheep blood (100 mL), and defibrinated horse blood (200 mL), all to be delivered bi-weekly on an automatic shipment basis. The contract period is one year from the date of award, with the option for the Director of the Division of Purchases to extend the contract for additional one-year periods at mutual consent. All shipments must arrive at the state destination during morning hours to allow for immediate refrigeration. Contractors must ensure products are delivered in first-class condition in accordance with good commercial practice, with all transportation charges and delivery costs included in pricing (F.O.B. destination). The state reserves the right to increase or decrease quantities on an as-needed basis throughout the contract period.
The estimated annual contract value is $5,000, though the actual value will reflect only quantities ordered during the contract term. Prices must remain firm for the duration of the contract, though price reductions may occur at any time and will apply to all subsequent shipments. The State of Maine requires contractors to accept the state's Procurement Card as a form of payment with no surcharges permitted regardless of payment method. The state is exempt from federal excise taxes and state sales taxes. Contractors must submit separate invoices within 30 days of delivery, each referencing the delivery order number, contract number, and vendor customer number. The Director of the Division of Purchases reserves the right to cancel the contract with thirty days' written notice or immediately if the contractor fails to comply with contract terms and conditions. Contractors must furnish the Director with detailed summaries of total purchases made under the contract as requested.
