Budget format suggestion: One budget for the overall project.

Background: The mission of CDC is to promote health and quality of life by preventing and controlling disease, injury, and disability. As the Nation's prevention agency, CDC must accomplish its mission by working with partners throughout the nation to monitor health, detect and investigate health problems, conduct research to enhance prevention, develop health policies to advocate sound public health programs as well as implement prevention strategies to promote healthy behaviors and foster safe and healthful environments.

Purpose and Scope of Work: The Centers for Disease Control and Prevention has identified the potential need for placement of public health professionals within each of the state health departments. Due to the additional burden being placed on the states due to the threat of terrorism as well as ongoing scientific/medical activities within the Nation CDC believes there may be a need to place CDC personnel on-site with the Health Departments. Due to the breadth of functions that must now be carried out on-site in the field, the need to call upon CDC State partners has dramatically increased to the extent that CDC must consider this option for immediate response preparedness. CDC believes increased responsibilities have the potential to continue to grow even more as the Nation enters challenging times.

Specific Task: Perform all required analysis, study and recommendation for potential placement of CDC Public Health Professionals at off-site locations (outside of CDC facilities) Deliverables:

Written Synopsis of preliminary results/findings. Within 90 to 120 days of the task order award.

Final Written Report of findings and recommendations. Eight months after award of the task order.

Formal Briefing to the executive management team on the results of review/analysis. Nine months after the award of the task order.
