About this file

This is a Vendor Packet form for RFQ 26-05 Furniture, Fixtures, and Equipment, a construction team project for furniture, fixtures, and equipment in Texas. The packet is designed as a fillable document to facilitate vendor participation in the bidding process. The form provides standardized fields and requirements for vendors to submit their proposals, ensuring consistency across all bidder responses. The document includes sections for vendor identification, product or service offerings, pricing information, and compliance certifications. Specific details regarding project scope, item quantities, delivery timelines, site visit opportunities, bidder conferences, proposal due dates, and contract award dates would be contained within the associated RFQ documentation and supporting exhibits referenced in the procurement materials.

The vendor packet establishes the framework for pricing submissions, vendor qualifications, and contractual terms applicable to the furniture, fixtures, and equipment procurement. The form likely contains set-aside provisions, insurance and bonding requirements, payment terms, and any applicable state or local compliance obligations. Vendors completing this packet must provide detailed pricing, product specifications, delivery schedules, and warranties as required by the solicitation. The packet serves as the primary submission vehicle through which interested vendors formally respond to the RFQ and demonstrate their capability and willingness to perform under the contract terms and conditions specified by the procuring agency.

CERTIFICATE OF INTERESTED PARTIES FORM 1295

In 2015, the Texas Legislature adopted House Bill 1295, which added section 2252.908 of the Government Code. The law states that a governmental entity or state agency may not enter into certain contracts with a business entity unless the business entity submits a disclosure of interested parties to the governmental entity or state agency at the time the business entity submits the signed contract to the governmental entity or state agency. The law applies only to a contract of a governmental entity or state agency that either (1) requires an action or vote by the governing body of the entity or agency before the contract may be signed or (2) has a value of at least $1 million. The disclosure requirement applies to a contract entered into on or after January 1, 2016.

The Texas Ethics Commission was required to adopt rules necessary to implement that law, prescribe the disclosure of interested parties form, and post a copy of the form on the commission's website. The commission adopted the Certificate of Interested Parties form (Form 1295) on October 5, 2015. The commission also adopted new rules (Chapter 46) on November 30, 2015, to implement the law. The commission does not have any additional authority to enforce or interpret House Bill 1295.

To complete the form online visit www.ethics.state.tx.us/whatsnew/elf_info_form1295.htm and follow any and all instructions.

A signed, hard copy of the original, completed Form 1295 is required to be submitted with any bid submission.
