About this file
This is a bid submission requirements document for Eastern Connecticut State University's Campus Dining Services contract opportunity in Connecticut. The document outlines the mandatory documentation that all bidders must include with their proposals to be considered for the University Dining Services contract, which covers all campus dining locations. While the document does not specify response dates, due dates, site visits, bidders' meetings, or contract term details, it establishes comprehensive compliance and administrative requirements that applicants must fulfill as part of their sealed proposal submission.
Bidders are required to submit nine distinct documents with their proposals, including a Bid Information Cover Sheet, Form 1 Gift and Campaign Contribution Certification (required for purchases valued at $50,000 or more annually), Form 5 Consulting Affidavit (also required for purchases of $50,000 or more), Form 6 OPM Ethics affirmation (required for contracts exceeding $500,000), Form 7 OPM Iran Certification (required for contracts exceeding $500,000), a completed Bidder Contract Compliance Monitoring Report to verify compliance with Connecticut General Statutes Sections 4a-60 and 4a-60a regarding equal employment opportunity and minority business enterprise inclusion, CHRO forms and notifications, a W-9 form, and a vendor profile form. The submission requirements reflect Connecticut's emphasis on ethical procurement, nondiscrimination compliance, and transparency in the contracting process, with particular attention to minority and women-owned business participation as subcontractors and suppliers.
