About this file
This document is a Statement Required Pursuant to KRS45A.395 from the City of Owensboro, Kentucky, specifically from their Purchasing Department located at 101 E. 4th Street. The form is a sworn statement required by Kentucky state law that bidders or offerors must complete as a prerequisite to being considered a responsible bidder. The statement requires the bidder to affirm, under penalty of perjury, that they have not knowingly violated any campaign finance laws of the Commonwealth of Kentucky.
The document includes blank spaces for the bidder to fill in their company name, representative's name, signature, title, and date. The key requirement is a declaration that neither the bidder nor anyone acting on their behalf has violated campaign finance laws, and that awarding a contract would not violate such laws. The form defines "knowingly" as being aware or should have been aware of the nature of one's conduct or existing circumstances. This document is part of the City of Owensboro's procurement process to ensure ethical bidding practices.
