About this file

This is a sworn statement form required pursuant to Kentucky Revised Statute 45A.395 for bidders and offerors submitting proposals to the City of Owensboro, Kentucky. The document serves as a prerequisite certification that must be completed before a bidder can be determined to be a responsible bidder, though it does not specify the particular products, services, project details, quantities, response dates, or contract terms as this is a standard compliance form used across multiple procurements.

The statement requires bidders to swear under penalty of perjury that neither they nor anyone acting on their behalf has knowingly violated Kentucky's campaign finance laws, and that awarding a contract to them would not violate these laws. The form includes signature lines for company representatives to complete with their name, title, company name, and date in 2025, and contains contact information for the City of Owensboro's Purchasing Department at 270-687-8431. This compliance document ensures adherence to state campaign finance regulations as part of the responsible bidder determination process.
