About this file
This is a sworn statement form required pursuant to Kentucky Revised Statute 45A.395 for the City of Owensboro's Purchasing Department, located at 101 E. 4th Street, P.O. Box 10003, Owensboro, KY 42302-9003. The document serves as a prerequisite form that any bidder or offeror must submit as part of their bid submission to be considered a responsible bidder. The form requires completion with the bidder's company information, including company name, authorized signatory details, title, and signature, with a date field showing 2025.
The statement requires bidders to swear under penalty of perjury that neither they nor anyone acting on their behalf has knowingly violated Kentucky's campaign finance laws and that awarding them a contract would not violate these laws. The document defines "knowingly" as being aware or should have been aware that conduct is of a particular nature or that certain circumstances exist. This compliance certification appears to be a standard requirement for all procurement processes with the City of Owensboro to ensure ethical contracting practices and adherence to state campaign finance regulations.
