About this file

This is a sworn statement form required by Kentucky Revised Statute (KRS) 45A.395 for bidders and offerors on Kentucky state and local contracts. The document serves as a prerequisite condition for establishing that a bidder is responsible and eligible to receive a contract award from the Commonwealth of Kentucky or its political subdivisions.

The statement requires the undersigned individual, acting in an official capacity on behalf of the bidding entity, to certify under penalty of perjury that neither they nor any person acting on behalf of the bidder has knowingly violated any provision of Kentucky's campaign finance laws, and that awarding the contract to the bidder will not violate any such laws. The term "knowingly" is defined statutorily to mean awareness or should-be awareness that conduct or circumstances are of a particular nature. The form includes blank spaces for the signatory to enter the date of execution (day and month of 2025), company name, authorized representative signature, title, and the office or title held by the individual executing the statement on behalf of the bidder or offeror.
