About this file
This is a campaign finance compliance statement form required by Kentucky statute KRS45A.395 for bidders or offerors submitting proposals to the City of Owensboro, Kentucky. The form is a sworn affidavit that must be completed as a prerequisite to being considered a responsible bidder, requiring the signatory to attest under penalty of perjury that neither they nor anyone acting on behalf of their organization has knowingly violated Kentucky's campaign finance laws, and that awarding a contract to their organization would not violate these laws. The document includes spaces for completion in 2025 and requires the company name, authorized signatory name, signature, and title to be provided.
The form serves as a legal compliance requirement rather than describing specific products, services, or contract terms, and appears to be a standard attachment that would accompany actual bid solicitations from the City of Owensboro. The document includes the city's purchasing department contact information and references the statutory definition of "knowingly" as being aware or should have been aware that conduct violates campaign finance provisions. No pricing terms, contract values, or specific procurement details are included in this compliance form.
