About this file
This document is a specification sheet and bid solicitation issued by the City of Fort Worth Purchasing Division for the procurement of fin form (Finland Form) panels for the Water Department on an as-needed basis under RFQt Reference #26-0024. The City of Fort Worth, which serves approximately 1.5 million customers across 4,000 miles of distribution mains and 4,000 miles of collection mains, requires fin form panels to reduce damage to customer property and utility infrastructure caused by excavation equipment outriggers during maintenance operations. The specified product must consist of hard, smooth Birch face veneers with Phenolic resin thermally fused to both sides, solid Birch inner piles, 14-Ply Arctic White Birch construction, true thickness of one inch, and dimensions of 4 feet by 8 feet, weighing approximately 110 pounds per panel, with full compliance to OSHA 29 CFR 1926 Subpart P standards for plywood in trench shoring. All materials must be delivered to the Fort Worth Water Department Field Operations at 1608 11th Avenue, Fort Worth, Texas 76102, during normal business hours of 8:00 a.m. to 5:00 p.m., Monday through Friday, excluding City-observed holidays. The initial contract term is one year with the option to renew for up to four additional one-year periods under the same terms and conditions, provided neither party submits notice of non-renewal at least 90 days prior to expiration.
Award will be made to the lowest responsive and responsible bidder whose bid price is reasonable and in the City's best interest, with the City reserving the right to make multiple awards, reject any or all bids, and award on individual items, item combinations, or overall bid basis. Bidders must submit completed RFQt-01 Forms, a completed Bid Table, and Small Business Enterprise Certificates if applicable. Pricing shall remain firm throughout the contract term and include all associated freight and delivery costs, with price adjustments permitted only on an annual basis at contract renewal upon submission of written justification and supporting documentation of cost increases at least 90 days prior to the requested effective date. Payment terms are Net 30 Days via direct deposit, with successful bidders required to register for electronic payment prior to providing goods or services. The solicitation is subject to the City of Fort Worth Small Business Program under Ordinance No. 27832-08-2025, Chapter 21 of the Code of the City of Fort Worth. The quantities listed in the bid table are estimates based on previous usage and do not guarantee future purchases; the City is obligated to pay only for materials actually ordered by authorized City employees and received and accepted by the City. Upon contract expiration, the successful vendor agrees to hold over under the same terms and conditions for up to 90 days to allow the City time to re-solicit an agreement.
