About this file
This is a Respondent Questionnaire attachment for a Request for Proposal (RFP) issued by the City of Detroit Office of Contracting and Procurement on behalf of the Construction and Demolition Department for roof replacement services. The project involves installing a Duro-Last Roof System at the CDD facility located at 6333 Huber Street, Detroit, Michigan. Proposals are due October 30, 2026. The scope of work requires an authorized Duro-Last installer to perform roof installation services at the specified facility. The questionnaire requires respondents to provide comprehensive information about their organization, relevant experience from the last ten years, subcontractor details, and leadership team qualifications to demonstrate their ability to successfully manage a contract of similar size and scope.
Respondents must complete detailed sections including company background information, contact details for the single point of contact and project manager, at minimum five prior relevant experiences with corresponding client references, subcontractor information and qualifications, resumes or CVs for leadership team members in PDF format, and an organizational chart for the office or unit providing services under the contract. The questionnaire also requires disclosure of any claims, lawsuits, contract terminations, debarred parties, Detroit resident employment details, and inquiries regarding Priority Hire Agreement participation and artificial intelligence use. All respondents must verify that proposed subcontractors are not on the City of Detroit's debarred and suspended suppliers list. The single-source specification for Duro-Last authorization restricts bidding to certified or authorized contractors.

| • | Professional licenses, accreditations, and/or certifications; |
| • | Education and/or training |
6. Organizational Chart
Instructions & Notices
The Respondent must provide an organizational chart for the business in a portable document format (.pdf).
If the business entity includes multiple offices/units across several regions/states/countries, the Respondent only needs to submit (an) organizational chart(s) for the office(s)/unit(s) that may provide services under this contract.
For example, 123 CONSTRUCTION Services solely operates in the State of Michigan. 123 CONSTRUCTION Services has offices in Detroit, Lansing, Grand Rapids, and Traverse City. However, 123 CONSTRUCTION Services only expects staff from the Detroit office to provide services under this contract. So, 123 CONSTRUCTION Services will only need to provide an organizational chart for the Detroit Office.
The Respondent must clearly indicate those positions (if any) in the organizational chart that are vacant/unstaffed.
If a Respondent has multiple positions of the same job title in the organizational tree, the Respondent is NOT required to list all the staff persons under that title. The Respondent may simply list the job/position title and the number of positions under that title. However, the Respondent must indicate the number of vacant positions under that title.
For example, 123 CONSTRUCTION Services has four Supervisory Technicians who each manage three Service Technicians and their Apprentices. One Service Technician position is vacant and two Apprentice positions are vacant. Therefore, 123 CONSTRUCTION Services may present that portion of their organizational chart as follows:
Supervisory Technician – X 4
Service Technician – X 12, 1 vacancy
Apprentice – X 12, 2 vacancies
