About this file

This is a Request for Application (RFA) cover sheet and application form from the Massachusetts Department of Mental Health (DMH) for Musical Instruments and Accessories Purchase Maintenance and Repairs Master Agreement (RFR Number: BD-25-1022-DMH08-8210C-113384). The RFA seeks vendors who can provide musical instruments, accessories, maintenance, and repair services. Bidders must have at least three years of experience in providing, servicing, or selling musical instruments, though exceptions may be considered for companies formed through recent mergers or acquisitions.

The pricing structure requires bidders to specify markup percentages for instrument purchases and replacement parts (both including delivery), as well as hourly labor rates for normal business hours, non-business hours, holidays, and weekends. A mandatory three-year warranty for parts and labor is required, or the manufacturer's warranty if greater, beginning after a 30-day operational acceptance period. Vendors must demonstrate financial solvency with three months of adequate reserves, no outstanding liabilities to government agencies, and must not be subject to any federal or state debarment orders or pending legal claims that could affect service delivery.
