About this file
This document is an Invitation for Bid (IFB) solicitation attachment package for Ocular Prosthesis Services and Devices issued by the California Department of Corrections and Rehabilitation/California Correctional Health Care Services (CDCR/CCHCS). The IFB solicitation number is SD25-00029 and seeks bids for custom prosthetic eyes, scleral cover shells, custom conformers, and related ocular prosthesis services including enlargement, reduction, and polishing/resurfacing. The services will be provided across four geographic regions within California's correctional institutions. Bidders must submit comprehensive bid packages that include all required attachments, such as the Bid/Bidder Certification Sheet, Bidder Declaration Form, various compliance certifications (including California Civil Rights Laws, Iran Contracting Act, and Darfur Contracting Act certifications), a Payee Data Record (STD 204), a Certificate of Good Standing, a GenAI Disclosure & Factsheet, a Bid Rate Sheet with specific line items and rate caps by region, and equipment specifications. Bidders must provide one rate for all institutions within each specified region and may use pre-identified subcontractors, with detailed subcontractor information required via the Bidder Declaration Form.
Pricing is structured by region with rate caps that vary significantly; Region 1, 2, and 3 institutions have identical caps of $8,000 for custom prosthetic eyes, $3,500 for scleral cover shells, and $700 for custom conformers, while Region 4 institutions have higher caps of $10,000, $5,000, and $1,000 respectively. Service rates similarly vary by region, with Regions 1-3 capped at $700 for enlargement, $500 for reduction, and $300 for polishing/resurfacing, and Region 4 capped at $1,500, $1,000, and $600 respectively. All bid rates must be at or lower than the identified caps and will remain in effect for the entire contract term and any extensions. The solicitation permits Disabled Veteran Business Enterprise (DVBE) incentive applications and Small Business Enterprise participation. Award will be based on the lowest responsible bid using a cost allocation formula that considers the contractor's rank and the number of agreements awarded. The State makes no express or implied guarantee of business volume, as service levels are subject to changes in the CDCR/CCHCS patient population.

California Department of Corrections and Rehabilitation (CDCR)/ California Correctional Health Care Services (CCHCS) Bid Rate Sheet
Bid Number: SD25-00029 Exhibit B-1
ATTACHMENT 13
A bid rate must be provided on each line item, in clear legible figures, in the space provided. Failure to provide the required bid rates shall be cause for rejection of bid.
Bidder must provide one (1) rate for all institutions listed within the specified region in which Ocular Prosthesis Services and Devices are bid on and will be provided to California Department of Corrections/California Correctional Health Care Services (CDCR/CCHCS) patients. Contractor must provide services to all institutions within the region. Failure to provide the required bid rates shall be cause for rejection of bid. While subcontractors may be used to comply with this requirement, pre-identified subcontractor(s) information must be submitted using the Bidder Declaration Form (GSPD-05-105). If additional subcontractors are added during the term of the Agreement, an updated GSPD-05-105 must be submitted to the CCHCS Direct Care Contracts Section Help Desk at: cchcshealthcarecontractshelpdesk@cdcr.ca.gov.
If awarded an Agreement, the bid rates for the goods/services submitted below shall remain in effect for the term of the Agreement, including any extension of the term through a formal amendment.
BID RATES MUST BE AT OR LOWER THAN THE IDENTIFIED BID RATE CAPS. ANY BID RECEIVED THAT
EXCEEDS THE RATE CAPS WILL BE REJECTED.
The Basis of Award shall be used to determine the lowest responsible bid. The lowest responsible Bidder will be the Rank one (1) Contractor on the bid matrix. The Agreement amount will be determined using a cost allocation formula that includes Contractor's rank and the number of Agreements awarded. In the case of a discrepancy between the Basis of Award and the bid rate per line item, the bid rates per line item shall prevail. However, if the Basis of Award is ambiguous, illegible, uncertain, or is omitted, the bid rates per line item shall be added to determine the Basis of Award.
Contractors with multiple Agreements for the same service at the same institution shall be obligated to provide service at the rate specified in Contractor's primary Agreement (i.e. agreement resulting from the first bid) until all obligations under that Agreement are satisfied before the rate in any subsequent Agreements can be used.
The only exception to this provision occurs when the rate in a subsequent Agreement is lower than those of the primary Agreement; the State then has the sole right to determine which rate will be applied.
The State does not expressly or by implication guarantee business, as services are subject to change depending on CDCR/CCHCS fluctuation in the patient population.
