Section 1: General Safety Rules:

1. These notes apply to window cleaning performed on all exterior faces of the building located at: 650 Capitol Mall, Sacramento, CA 95814. The OPOS sheets illustrates the use of the window washing equipment supplied by Tractel. All other equipment must be provided by the contractor performing such work.

An Ultimate Recommendation

Never neglect means to improve safety. Due to the risks inherent in the use of suspended scaffold systems, the supplier strongly recommends that every installation be equipped with four wire system. Four wire rope(s) systems may have provisions on the platform for attaching the operator's harness lanyard. The lanyard should have a two-way acting rope grab to prohibit sliding down the platform should it reach an unstable angle.

2. This building has a height of 128ft.

3. All users using window washing equipments supplied/manufactured by Tractel Ltd. must be trained by Tractel Ltd. personnel. Only experienced personnel who are adequately and fully trained to use the necessary equipment shall perform the Window Cleaning Procedure outlined on CAL-OSHA Title 8 CCR Article 5, 6 and 24, and specifically paragraph 3282, "General Requirements for Window Cleaning Operators". Operators shall be equipped with walkie-talkies to assure a 2-way communication between the Powered Platform and either to Building Security (at ___-___-____), or Building Manager (at ___-___-____) in case of emergency.

4. Check-In Procedure It is necessary for the scaffold operators/contractors to ascertain who the building's designated representative/building engineer will be and check in with that person before beginning work. The building's designated representative should know about the nature of work, location and the time of day the work will be performed. The building representative, along with the operators, will determine the drops to be done as well as the sequence. The building's designated representative should also review all CAL-OSHA acceptable emergency rescue procedures with the scaffold operators/ contractors.

"Property Management" shall provide to the "scaffold operator/contractor”

a) Tractel's operating and maintenance manual for the buildings dedicated equipment

b) Operating Procedure Outline Sheet “OPOS”

The scaffold operator/ contractor shall not operate or use any equipment before thoroughly reading and understanding the OPOS. Notify building management and do not use the equipment if the OPOS is in any way unclear or if the equipment is damaged or questionable.

5. All portions of the building are accessed using the equipment. This equipment is permanently dedicated to the building. The operators shall refer to manufacture's printed installation, operation and maintenance instructions prior to using the equipment. The instructions and guidelines by the manufacturer of the equipment shall be STRICTLY adhered to. Never alter, remove or substitute parts or components of the window washing equipments except as directed by the manufacturer.

6. Operators shall stay off all parapet walls, canopies, ledges, sills wherever applicable, and roof protection must be provided. Sitting or Standing on the parapet walls, handrails or on the facade is STRICTLY PROHIBITED. Failure to comply with the following instructions could cause serious equipment damages, injuries or death! Failure to comply with the following instructions could result in the removal of the operator from the job site.

7. All workers must wear an approved personal fall arrest device and must attach the safety line of their body harness to a "horizontal lifeline" in the platform PRIOR to making any service drop (by: Others).

8. Fire/Corrosive Chemicals: Wire ropes and other parts of the platform are susceptible to serious damage IF EXPOSED TO FIRE, UNDUE HEAT, CORROSIVE ATMOSPHERIC CONDITIONS, OR TO CORROSIVE CHEMICALS. Notify your employer, dealer or the manufacturer immediately if exposure occurs or is suspected and DISCONTINUE USE of the platform. Do not carry flammable liquids on the working platform.

9. Wind Speed Limit Operation of the "modular powered platform" is PROHIBITED WHEN THE WIND SPEED EXCEEDS 25 MPH. All operators must continually monitor the wind speed by using an anemometer (by: Others).

10. Unscheduled maintenance occurs with less frequency than scheduled maintenance. This section of the OPOS describes the application of scheduled maintenance procedures and does not address Unscheduled Maintenance rigging of support apparatus.

11. Check-Out Procedure Upon completion of the day's work, the scaffold operators will check out with the building representative and advise that they have finished for the day.

EMERGENCY Operating Procedure:

It is important that there are appropriate procedures to be followed in case of an emergency. The building's emergency operating procedures must be read and understood by all operators performing building maintenance. All operators shall familiarize themselves with the building’s exit routes, access doors, emergency escape routes and alarm system before performing building maintenance. In addition, maintenance operators shall be knowledgeable of escape routes and procedures from the area and equipment they will occupy during the course of their work. Before any OPOS maintenance work begins, the Building Representative (at ___-___-____) shall be notified of the nature, location and time of day work will be performed.

Problems with the scaffolding, if any, should be reported to the building representative immediately. Any incident or accident with regard to the scaffolding or the building must also be reported to the building representative immediately.

General Emergency Operator(s) should immediately notify Building Representative (at ___-___-____) and explain the nature of the problem and their location. Unless instructed to do otherwise, run the modular powered platform and other equipment to a safe level, exit the equipment and, if necessary, perform safe egress from the building. Operator(s) should notify Tractel Ltd. Representative (Kirit Parmar at 415-264-8376) about the emergency for Tractel supplied equipment. Manufacturers of other equipments should also be notified.

CATEGORY "A": Power Failure Operators are stranded or stalled at their workstation on side of the building due to conditions such as power failure but are in no imminent or immediate danger. During a Category “A” Emergency, follow the General Emergency operating procedure.

Depending on the building representative’s instructions, the operator(s) may wait until the power of the modular powered platform is restored or they may run the equipment in its manual mode to a safe level and exit the equipment. There may be cases where the building representative is not able to resolve the Category “A” Emergency.

CATEGORY "B": Equipment Malfunction Operators are incapable of ascent or descent due to equipment failure but are in no imminent or immediate danger. During a Category “B” Emergency, follow the General Emergency operating procedure. Depending on the building representative’s instructions, the operator(s) will descend the equipment in its manual mode to a safe level and exit the equipment.

CATEGORY "C": Imminent / Immediate Danger due to Equipment Malfunction Operators are incapable of any movement due to equipment failure and remaining on the scaffold platform will expose the operator(s) to imminent or immediate danger.

During a Category “C” Emergency, the operator(s) must do whatever they can to stabilize the situation, and then follow the General Emergency operating procedure.

Following the building representative’s instructions, the window at that location must be removed and the platform operator(s) will perform a safe egress from the platform.

Upon resolution of the emergency, the equipment must be thoroughly inspected.

Scaffold platforms are equipped with emergency descent controls in case of a Category "C" Emergency. All operators using the scaffold platform supplied by Tractel must be trained in proper rigging and use of descent device(s) by Tractel Ltd.

personnel. Scaffold operators shall inspect the descent devices available on the scaffold before performing the initial drop and special attention will be given to ensure that the scaffold platform is capable of a safe descent from the highest point.

Refer to Inspection section in the Operating and Maintenance Manual provided by Tractel.

Personal Safety Device for a "Scaffold Platform" Emergency Rescue

Additionally, the work operator shall have extra equipment necessary for rescuing a stranded or incapacitated operator while accessing the building's façade. The operator shall prevent ropes from becoming kinked during operation. Additional equipment that may be needed by the rescuer, include.

i. Lifeline iv. Rope Grab, Separate Lifeline & Lanyard

ii. Descent Device and Carabiner v. Sharp Knife iii.2-Way Radio Communication vi. Positioning Line with Rope Grab Note: KNOTS in ropes are PROHIBITED.
