STATEMENT OF WORK
Telecommunication Services
STATEMENT OF WORK
| 1. | Purpose | The Health and Human Services (HHS), Indian Health Services (IHS), Supai Health Station, located in Supai, Arizona, has a requirement for contractor to provide 24/7/365 services for local exchange access commercial network service supporting subscriber services to the Supai Health Station. |
| 2. | Phone and Other Telecommunications Services Shall Include: |
Contractor to provide a comprehensive set of local exchange access services, analog, and/or digital access transport services, analog, inter- and intra-Local Access Transport (LATA) interconnect function.
The Contractor shall provide all transmission facilities, equipment, materials, maintenance, test equipment and design layout needed to perform and provide local access and transport services and functions at the designated demarcation points listed.
Telecommunication services to include:
7 Analog dial tone / access lines to long distance service Contractor must be able to retain existing telephone number and number blocks currently in use at Supai Health Station. Provider must also be able to reproduce existing Voice T1 circuits and/or POTS lines currently in use at Supai Health Station.
Contractor shall provide Telecommunication services at listed sites in B.5. The service shall be in accordance with all Federal, State, Local, Tribal and IHS regulations and shall be made with minimal divergence from the schedules established and with minimum disturbance and maximum protection of government personnel, property and general public.
