Attachment 0 Synopsis/Solicitation 75H71221Q00027 Supai FAX Services Colorado River Service Unit
The Contractor shall provide provide 24/7/365 services for local exchange access commercial network service supporting subscriber services to the Supai Health Station, Supai, AZ.

B. Request for Information:
All information requests or any correspondence concerning this requirement shall be provided by email only to Nestor.Noble@ihs.gov. Subject Title of email is 75H71221Q00027 Supai FAX Services. Please do not contact anyone else regarding this requirement.
C. Solicitation CLOSING: All quotation materials shall be received NO LATER THAN August 10, 2021 by 11:59 am Pacific Standard Time. LATE QUOTES WILL NOT BE ACCEPTED, except as detailed in FAR 52.212-1(f), Late submissions, modifications, revisions, and withdrawals of offers.
D. All questions regarding this solicitation shall be submitted by July 28, 2021.
E. The following factors shall be used to evaluate proposals:
1. Technical capability: The offeror shall submit a capabilities statement and include a detailed narrative of how the contractor will accomplish the deliverables.
1. Past Performance Information: The offeror shall provide information concerning recent (within the past three years) and relevant past performance in performing at least three and no more than five contracts of similar size and scope. In addition to the information submitted with the offer, the Contracting Officer shall consider knowledge of and previous experience with the supply or service being acquired, customer surveys and the Government wide Past Performance Information Retrieval System (PPIRS) at www.ppirs.gov or other information.
1. Price: Offerors shall provide prices in the CLIN structure.
The Government intends to evaluate quotes and award without negotiations. The initial proposal submitted must contain adequate information to ascertain the contractor’s ability to comply with all requirements of the solicitation and it should contain the offeror’s best terms from a price and technical standpoint. The Government reserves the right to conduct negotiations with one or more offerors, if determined to be necessary by the Contracting Officer.
b) Options. IAW the provision at FAR 52.217-5, the Government will evaluate offers for award purposes by adding the total price for all options including the option under FAR 52.217-8, to the total price for the basic requirement. Evaluation of options will not obligate the government to exercise options. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced.
Offerors are advised that the Option to Extend Services Clause at 52.217-8 is included in the solicitation and will be included in the awarded contract. When proposing prices for base and option years, Offerors should consider that the government may choose to exercise the option to extend services at the end of any period of performance (base or option periods), utilizing the rates in effect during the period of performance in which the option is exercised. For purposes of evaluation, the government assumes that any price proposed for base and option periods reflects the possibility that a period of performance may be extended.
Offerors shall include a price for the six month option for evaluation purposes only .
1. A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
