About this file

This document is a Past Performance Questionnaire (PPQ) that the Defense Health Agency (DHA) Western Markets Contracting Division is using to evaluate an offeror for a U.S. Government contract. The questionnaire requests the evaluator to rate the offeror's past performance on factors such as relationship with the client, management of subcontractors, ability to hire and retain qualified employees, quality of work, schedule and cost control, compliance with labor and safety standards, and timely submission of required documentation. The evaluator is also asked to indicate whether any negative performance issues occurred and whether they would award another contract to the offeror. The results of this questionnaire will be used to assess the offeror's suitability for the contract opportunity, which is for fire extinguisher service support services for the Naval Medical Center San Diego (NMCSD).
