About this file

This is a Bid Rate Sheet for biohazardous medical waste removal services submitted to the California Department of Veterans Affairs (CalVet) for the Veterans Home of California in Yountville. The contractor shall provide all labor, materials, staff, transportation, licenses, permits, and other necessary expenses to deliver biohazardous waste pickup services at the facility located at 100 California Drive, Yountville, California 94599. Services include scheduled pickups for four waste categories: biohazardous medical waste (estimated 100 pounds per pickup, 157 pickups over the contract term), pharmaceutical waste (80 pounds per pickup, 157 pickups), chemotherapeutic waste (20 pounds per pickup, 157 pickups), and RCRA waste (80 pounds per pickup, 157 pickups). The rate sheet also includes provisions for unscheduled emergency pickups within 24 hours of request, with an estimated 20 such pickups anticipated during the contract term.

The rate sheet requires proposers to enter all-inclusive pricing per pound for waste types and per pickup for emergency services, with all fields mandatory and zero-dollar amounts permissible only if services are provided at no cost to the State. Contractors must also provide pricing for collection containers and supplies, including 8-gallon and 12-gallon pharmaceutical waste containers, 2-gallon chemotherapeutic waste containers, and 2-gallon and 8-gallon RCRA containers, with 157 containers of each type estimated during the contract term. The Basis of Award is the Total Amount of Agreement, calculated by summing all rate sheet line items. The solicitation is designated as DVBE (Disabled Veteran Business Enterprise) only, restricting eligibility to certified disabled veteran-owned businesses. Submission of the completed and signed rate sheet is mandatory, and any alteration to the document will result in rejection of the entire quote.
