About this file
This document is a Draft Past Performance Questionnaire (PPQ) for the FEMA National Cross Dock & Incident Base Support Contract. The questionnaire is designed to collect reference information from previous clients about a contractor's performance, focusing on three key performance elements: quality of services, customer satisfaction, and timeliness of performance. The form requires evaluators to rate the contractor's performance on a scale from unsatisfactory to satisfactory and includes a section asking whether the evaluator would do business with the contractor again.
The PPQ is part of a pre-solicitation for a multiple award Indefinite Delivery Quantity (IDIQ) contract supporting FEMA's Cross Docks operations. Completed questionnaires must be submitted electronically to Contracting Officer Ed Ruud and Contract Specialist Marcia Irizarry Snyder by the proposal closing date. The document emphasizes confidentiality and explains that the submitted information will be used to evaluate the offeror's past performance capabilities for the upcoming solicitation.
